Purpose: To determine the relationship between pretreatment serum squamous cell carcinoma (SCC) antigen and Cyfra 21-1 levels, and survival in patients with invasive squamous cell carcinoma of the cervix.Materials and Methods: One hundred and one cervical squamous cell carcinoma patients were included. Pre-treatment levels of serum SCC antigen and Cyfra 21-1 were measured, with a 5 year minimum follow up. Thirty two recurrent disease (RD) patients were compared to 99 non-recurrent disease (NRD) patients with respect to tumor markers, FIGO stage, lesion size, lymph node status, and parametrial involvement.Results: Pre-treatment serum SCC antigen and Cyfra 21-1 levels were significantly higher in the RD group (p＜ 0.001). Combined serum SCC antigen and Cyfra 21-1 levels showed higher sensitivity for prediction of recurrence (90.6% ). Pre-treatment SCC antigen and Cyfra 21-1 levels showed correlation with high FIGO stage, large lesion size, lymph node status, and parametrial involvement (p＜0.001). Normal pre-treatment levels of SCC antigen and Cyfra 21-1 showed a 5-year survival rate of 93% and 90% respectively, while elevated levels showed significantly decreased survival rate of 63% and 59% , respectively (p＜0.001). Odd ratio for cumulative survival rates were 6.87 for SCC antigen, and 5.07 for Cyfra 21-1 (p＜0.001).Conclusion

The dysregulation of proliferating and apoptotic processes is a common feature in cancerogenesis. Thus, apoptotic products released into blood circulation are suggested as promising markers for the early cancer detection. However though sensitive assays are available, the lack of organ- and tumor-specificity limits the usefulness of most apoptotic parameters for screening purposes. However, they seem to be valuable for the prognosis and the prediction of response to systemic chemo- or radiotherapy in cancer disease.Here, the relevance of diverse circulating apoptotic markers is reviewed for the clinical management of patients with lung cancer. Among those promising markers are ligands and receptors of the FAS-system, members of the intracellular caspase cascade, cleaved apoptosis substrates such as cytokeratines, nucleosomal DNA, and apoptosis modulators like survivin. The review discusses their role for diagnosis, prognosis and therapy monitoring of lung cancer disease.

ZusammenfassungDer Nachweis zytokeratin-positiver Zellen im Knochenmark von Patientinnen mit soliden Malignomen gilt als Indikator für eine hämatogene Disseminierung von isolierten Tumorzellen (ITC) und ist mit einem erhöhten Risiko für das Auftreten von Fernmetastasen assoziiert. Es war Ziel dieser Studie, die Inzidenz und prognostische Relevanz von ITC im Knochenmark von Patientinnen mit einem Zervixkarzinom FIGO I ± II zu überprüfen. Methodik: Im Zeitraum von Mai 1994 bis Januar 2001 wurde an der I. Frauenklinik der Ludwig-Maximilians-Universität München prospektiv bei 120 konsekutiven Patientinnen mit primärem Zervixkarzinom vor Beginn der operativen Primärtherapie eine Knochenmarkpunktion durchgeführt. Der Nachweis isolierter Tumorzellen im Knochenmark wurde mit dem Panzytokeratinantikörper A45-B/B3 erzielt. Die Patientinnen wurden anschlieûend im Median 43 (min-max, 1 ± 85) Monate nachbeobachtet. Ergebnisse: ITC wurden bei 32 von 120 Patientinnen (27%) detektiert. Der Nachweis von ITC korrelierte weder mit dem FIGO-Tumorstadium (p = 0,87) noch mit dem pelvinen und paraaortalen Lymphknotenbefall (p = 0,19), dem histopathologischen Grading (p = 0,42), dem histologischen Subtyp des Karzinoms (p = 0,45), Lymphangiosis (p = 0,91) und Hämangiosis (p = 0,46), und dem Menopausenstatus (p = 0,24). Bei 17 Patientinnen (14,6%) wurde während der Nachbeobachtungszeit ein Rezidiv Abstract Purpose: Isolated cytokeratin-positive tumor cells in the bone marrow of patients with solid tumors are a marker of hematogenous spread and a risk factor for distant metastases. We examined the incidence and prognostic relevance of isolated tumor cells in the bone marrow of women with FIGO stage I ± II cervical cancer scheduled for surgery. Methods: Bone marrow aspiration was performed in a series of 120 consecutive women with cervical cancer before the beginning of surgery between May 1994 and January 2001. The aspirates were stained with the pancytokeratin antibody A45-B/B3. The median follow-up was 43 months (range, 1 ± 85). Results: Isolated tumor cells were detected in the bone marrow of 32 patients (27%). The presence of tumor cells did not correlate with FIGO stage, pelvic and paraaortic lymph node status, histologic grade, histologic subtype, lymphatic or vascular space involvement, or menopausal status. Seventeen women (15%) developed recurrence. The rate of locoregional recurrence was similar in women with or without tumor cells in the bone marrow, but distant recurrence was more common in women with positive bone marow (9.4 % vs. 2.3 %, P = 0.085). Kaplan-Meier analysis for survival without distant recurrence yielded a mean survival of 77.6 months (95% CI, 70.4 ± 86.5) for women with and 80.3 months (95% CI, 77.8 ± 82.9) for women without tumor cells in the bone marrow (P = 0.11).
