Severe heart failure secondary to 5-fluorouracil and low-doses of folinic acid: Usefulness of an intra-aortic balloon pump

Abstract: Intravenous 5-fluorouracil in combination with low doses of folinic acid can induce severe nonischemic heart failure. In such a case, an intra-aortic balloon pump could be useful by providing left ventricular function support when inotropic agents and vasopressors fail to restore normal hemodynamics.

“…Additionally, the presence of these features in the setting of drug use, excludes the diagnosis of SCM, per Gothenburg Criteria 38. A similar presentation of reversible global hypokinesis associated with 5-FU has been previously described,39–45 and these cases do not meet the strict definition of SCM (table 1). Thus, these reports of 5-FU-related global hypokinesia, make us think about our current understanding of the underlying mechanism for this cardiotoxicity.…”

“…In one report, the incidence of cardiomyopathy with left ventricular dysfunction was estimated to be 2% [3]. Severe cardiotoxic manifestations have been reported in case reports, including coronary dissection, ventricular tachyarrhythmia, cardiogenic shock (requiring intra-aortic balloon pump and extracorporeal membrane oxygenation support), and sudden cardiac death [32][33][34].…”
