Severe meningococcal serogroup W sepsis presenting as myocarditis: A case report and review of literature

Abstract: The global incidence of invasive meningococcal disease due to serogroup W (MenW) has risen over the last decade. The following case emphasises the atypical features of MenW meningococcaemia, which included myocarditis, a rare but important complication. It also highlights the potential novel role that cardiac magnetic resonance imaging can provide in the diagnosis of MenW myocarditis. Complications of these infections can be avoided with early recognition and susceptibility testing to prevent the use of inappr… Show more

“…Bacterial agents leading to myocarditis include pathogens such as Corynebacterium diphtheria [ 29 ], Beta-haemolytic streptococci [ 30 ], Meningococci [ 31 ], Salmonella typhior paratyphi [ 32 , 33 ], Borrelia burgdorferi [ 34 , 35 , 36 ], Mycoplasma pneumonia [ 37 ] and Chlamydia psittaci [ 38 ]. Corynebacterium diphtheria infection is now less frequent in western regions [ 39 ], while it is still a major public health problem in many underdeveloped countries and perhaps the most common cause of myocarditis worldwide [ 40 ].…”

“…This follows increased rates of MenW in the United Kingdom, Europe, South America and the United States 2,8 . The most common manifestations of IMD are meningitis or septicaemia; 9 however, a higher proportion of cases of IMD due to MenW has been described with altered clinical presentations, with atypical features such as pneumonia, septic arthritis and myocarditis 7,8,10 . It is not clear how the overall clinical presentation of IMD has been affected by this change in circulating serogroups.…”
