Aim Postoperative cognitive dysfunction is often observed in older patients. Previous reports described the link between postoperative cognitive dysfunction and general anesthetics, such as sevoflurane, but the exact mechanism remains unclear. We therefore sought to characterize the effects of sevoflurane on hippocampal‐dependent cognitive functions, as well as hippocampal plasticity, and to delineate the underlying mechanisms. Methods Behavioral assays including the novel object recognition test and the Morris water maze test were carried out to assess the cognitive performance of control and sevoflurane‐exposed mice. Electrophysiological recordings were carried out to evaluate the sevoflurane‐induced changes of synaptic plasticity in the hippocampus. Furthermore, western blot assay was utilized to quantitatively assess the altered protein expression resulting from sevoflurane exposure. Results Sevoflurane anesthesia impaired cognitive functions, as well as metabotropic glutamate receptor‐dependent long‐term depression, through elevated surface expression of small conductance calcium‐activated potassium type 2 channels. Blockage of calcium‐activated potassium type 2 channels reversed the sevoflurane‐induced deficits at both cellular and behavioral levels. Conclusions Sevoflurane anesthesia impaired metabotropic glutamate receptor‐dependent long‐term depression and thereby affected cognitive functions in old mice. Inhibitory modulators of calcium‐activated potassium type 2 channels might prevent cognitive decline elicited by sevoflurane. Geriatr Gerontol Int 2019; 19: 357–362.

Abstract. The aim of the present study was to evaluate the safety and efficacy of various general anesthesia regimens during endoscopic variceal ligation (EVL) and endoscopic variceal sclerotherapy (EVS). A total of 123 patients with American Society of Anesthesiologists physical status III and IV, aged 40-70 years, undergoing general anesthesia for EVL and EVS were randomly divided into two groups: Sevoflurane anesthesia (group S; n=60) and propofol anesthesia (group P; n=60). Vital signs, particularly heart rate (HR) and mean arterial pressure (MAP), were monitored. The designated time points were as follows: 5 min before induction (T0), and 1, 5, 10, 15, 20, 25 and 30 min after intubation (T1, T2, T3, T4, T5, T6 and T7, respectively). Time intervals were recorded, including recovery time and extubation time. Following surgery, the observer recorded the Ramsay sedation scale (RSS) score and the visual analogue scale (VAS) score. Adverse reactions were noted. Results demonstrated that there were significant differences in MAP between the two groups at T2, T3, T5, T6 and T7 (P<0.05). There was a significant difference in HR between the two groups at T2, T3 and T4 (P<0.05). Recovery time and extubation time in group P were significantly longer than those in group S (P<0.05; 18.38±2.25 min vs. 14.57±1.04 min and 21.70±2.70 min vs. 15.83±0.88 min, respectively). The rate of ephedrine injected was 58.3% (35/60 patients) in group P vs. 28.3% (17/60 patients) in group S (P<0.05). There was a significant difference in the RSS score between the two groups 5 min after extubation (P<0.05). VRS scores demonstrated that anesthetists and patients were significantly more satisfied with the procedure in group S than in group P (P<0.01). In conclusion, the superiority and special clinical value of inhalational anesthesia has been demonstrated during EVL and EVS attributed to stable hemodynamics and high quality of anesthesia recovery in the present study.

Perfil clínico-epidemiológico de pacientes cirúrgicos com ressecamento ocular e risco de ressecamento ocularClinical-epidemiological profile of surgical patients with ocular dryness and risk of dry eye RESUMO Objetivo: determinar o perfil clínico-epidemiológico de pacientes cirúrgicos com ressecamento ocular e com o diagnóstico de enfermagem Risco de ressecamento ocular, no período pós-operatório. Métodos: estudo transversal, com 82 pacientes em pós-operatório. Para análise, utilizaram-se as frequências, medidas do centro de distribuição e respectivas variabilidades. Resultados: prevalência equivalente entre o ressecamento ocular e o diagnóstico de enfermagem. A média de idade foi 57,7 anos para pacientes com ressecamento ocular e 59,6 anos para aqueles com o diagnóstico de risco. Em ambos os grupos, existiu predominância do sexo feminino, cirurgias cardiovasculares e uso de anestésico geral. O tempo de procedimento cirúrgico obteve mediana de 105 minutos para pacientes com ressecamento ocular e 67,5 minutos para aqueles com o diagnóstico de risco. Conclusão: predominância de mulheres em meia idade submetidas a cirurgias eletivas de motivos cardiovasculares para ambos os grupos, entretanto, com tempo de procedimento cirúrgico distinto. Descritores:ABSTRACT Objective: to determine clinical and epidemiological profile of surgical patients with ocular dryness and to the Nursing diagnostic and risk of ocular dryness, in the postoperative period. Methods: cross-sectional study, with 82 patients in the postoperative period. For analysis, it were used frequencies, the distribution centers' measures and respectives variabilities. Results: equivalent prevalence between ocular dryness and the nursing diagnosis. The average age was 57.7 years for patients with ocular dryness and 59.6 years for those with risk diagnosis. In both groups, there was a predominance of females, cardiovascular surgeries and general anesthetic usage. The length of the surgical procedure averaged 105 minutes for patients with ocular dryness and 67.5 minutes for those with risk diagnosis. Conclusion: predominance of women in middle age undergoing elective surgeries by cardiovascular reasons for both groups, however, with distinct time of surgical procedure.
