Sex hormone levels and change in left ventricular structure among men and post-menopausal women: The Multi-Ethnic Study of Atherosclerosis (MESA)

Abstract: A more androgenic profile (higher free testosterone and lower SHBG levels) is associated with a greater increase in LVM in men and women and greater increase in M:V ratio in women over the course of 9 years.

“…Consistent with this, we previously found in this same cohort that a more androgenic sex hormone profile was associated with increased left ventricular mass in postmenopausal women. 19 We did not find an association of estradiol with aortic stiffness in women, but endogenous estradiol levels are low in women not on HT following the menopausal transition. Exogenous estrogen administration in the form of HT may have favorable effects on arterial stiffness.…”

“…Total testosterone and DHEA were measured directly with radioimmunoassay kits, and SHBG was measured by chemiluminescence enzyme immunometric assay using Immulite kits (Diagnostic Products Corporation, Los Angeles, CA). 19 Free testosterone (reported as percent of total testosterone) was calculated using the Sodergard method. 20 The intra-assay coefficients of variation for total testosterone, SHBG, DHEA, and estradiol were 12.3%, 9.0%, 11.2%, and 10.5%, respectively.…”

“… 40 , 43 For postmenopausal women, estrogens decrease rapidly, and they are under a more androgenic pattern of sex hormones. 44 , 45 Although men also have age-related hormonal changes, the sex hormonal changes are gradual compared with menopause. 46 Thus MGD would be more severe in older men.…”

“…These data are extremely important since both cohorts of patients in that study had a very high rate of secondary prevention medications after one year (i.e., beta-blockers 90-95% and ACE inhibitors/angiotensin II R-blockers 93-97% respectively in the two studies). Interestingly, sex hormones are associated differently with the morphology of the remodelled heart wall, and a more androgenic profile is responsible for a greater imbalance in the heart mass/volume ratio in females than in males [70].…”
