Purpose: To grasp the scope of sexuality and fertility within the lived experiences of comfort and discomfort among allogeneic stem cell transplant survivors. Methods: We employed a qualitative approach through van Manen's phenomenology of practice. The phenomenon was revealed via phenomenological interviews, incorporating narratives and illustrative episodes that captured the lived experiences of 20 survivors. We gathered descriptions of these experiences from participants from July 2020 to May 2021. During our phenomenological reflection on these accounts, we adhered to stages like epoché, reduction, and vocative, as outlined by van Manen (1). Results: The exploration of comfort within the realms of sexuality and fertility yielded several themes: desire and the relationship; immunosuppression as a limitation on sexual activit; the body does not correspond to desire; the future: fertility and the couple's relationship; the challenges of disclosure and intervention. Conclusion: The findings indicate that survivors have long-term challenges with sexual expression and activity, stemming from the physical constraints of GVHD, perceptions of their immunosuppression, and the significance of their partnership, all of which translate into requirements for nursing care. Implications for Cancer Survivors Considering the experiences of these participants and the observed comfort linked to the sexuality of AHSCT survivors, we can contemplate the care practices and recognize sexuality as a field of action for nurses in this context to enhance comfort for these individuals.

Stem cell transplant proved its efficacy in increasing the survival rate among young patients diagnosed with hematological malignancies. A transplant conditioning regimen is particularly destructive on the genital system, often determining premature ovarian failure, accompanied by vulvovaginal atrophy and sexual dysfunctions. The aims of the present study were, first, to evaluate sexual dysfunctions among transplanted women, using clinical examination and the female sexual function index (FSFI), and second, to determine their impact on a couple’s relationship. A prospective observational comparative study was performed and included 38 patients who underwent allogenic stem cell transplant (SCT) procedures for different hematological malignancies and 38 healthy patients (control group). This study included baseline evaluation, one-year, and three-year follow-up visits. In addition to anamnesis and medically obtained information, FSFI was evaluated to determine the impact of gynecological damage in a subjective manner. In the study group, vulvovaginal atrophy was diagnosed in 76.32%, with subsequent sexual dysfunctions in 92.10% of patients, based on FSFI scoring. Even though the results improved throughout the study, at the last visit, mild vulvovaginal atrophy was diagnosed in 81.58% of patients, and the FSFI score was abnormal for 21.05%. When compared to the control group, both sexual dysfunctions and FSFI results were considerably impaired, with statistical significance. There is a confirmed negative impact of sexual dysfunctions and self-declared FSFI on couple/marital status and couple relationships, with statistical significance, at the last visit. In conclusion, anatomical, functional, and psychological difficulties are a reality of long-term survivors after a stem cell transplant. They should be addressed and assessed equally to other medical conditions, as they may determine serious consequences and impact the sexual quality of life and the couple’s relationship.

Asthenic syndrome is one of the most common conditions in the clinical practice of medical specialists having various therapeutic spheres The genesis of asthenic disorder is diverse and can be represented by cerebrovascular pathology, metabolic disorders, hypoxia of various origin, as well as intoxication. Asthenic syndrome can be diagnosed in 70–75% of patients with oncological pathology at various treatment stages. The article presents a case study concerning the asthenic syndrome progression in a patient with acute lymphoblastic leukemia (ALL) and its successful correction by metabolic therapy course. Case study pattern consist in the occurrence of this syndrome in the setting of multiple factors (infectious, metabolic, associated with intracranial hemorrhage) in the early period after hematopoietic stem cell transplantation (HSCT). The article provides a pathogenetic justification for the therapy indication with antihypoxants (cytochrome C) in patients with oncological pathology aimed at the asthenic syndrome correction in this patient cohort. Cytochrome C is a heme- containing protein of natural molecular origin. It is a participant in the process of cellular respiration and a regulator of redox processes in tissues.
