This review examines the psychologically oriented literature on adolescent contraceptive use. Descriptive studies, individual difference studies, and theory-based studies from family planning, medical, and applied journals are covered, as well as studies appearing in psychology journals. The research covered suggests that adolescents are largely uninformed about reproductive physiology and about many methods of contraception. They have negative attitudes toward contraception generally and toward using contraceptives. The widespread nonuse of contraception by sexually active adolescents is not irrational, given their levels of information and their beliefs and attitudes. Psychological models using cognitive, emotional, and developmental factors have been used to predict and explain these behaviors, and they hold promise for future research.Someday you'll find a chap you'll love with your whole heart and who will adore you as wholeheartedly. When that time comes you'll be glad if you've kept the intimate parts of your body for him. You'll understand then how cheap it is to allow yourself to be handled for a mere sexual thrill. (Schultz, 1955, p. 192) Birth control is probably the most important thing for young people to know about sex, because to enjoy sex and make it part of caring, it is absolutely essential to be sure you won't produce a baby unless you want one. (Comfort & Comfort, 1979, p. 70) Attitudes toward adolescent sexuality have changed greatly during the past three decades. There have been even more dramatic changes in contraceptive technology during the same period, including the introduction of oral contraceptives, intrauterine devices (ILTDs), and, most recently, the contraceptive sponge. Despite these changes, adolescent pregnancy rates continue to rise. In 1978, 1.14 million American teenagers-over 10% of all teenage women-became pregnant. PurposeTo explore adolescent health seeking behavior during pregnancy and early motherhood in order to contribute to health policy formulation and improved access to health care. This will in long‐term have an impact on the reduction of morbidity and mortality among adolescent mothers and their newborns.MethodsThis was a qualitative study that employed focus group discussions (FGDs) among adolescent girls (10–19 years) and key informant (KI) interviews with health workers. Age for FGD participants ranged from 16 to 19 years. The FGD participants were recruited while seeking antenatal care for their first pregnancy or immunization service for their first child, not being older than 6 months. Six health facilities were selected. Key informants were purposefully selected on the basis of being in‐charge of maternity units. Thirteen FGDs comprising of a total of 92 adolescent girls were conducted. The FGDs were held with homogeneously constituted categories; married pregnant adolescents (5), unmarried pregnant adolescents (3) and married or not married adolescents with children (5). Semi structured interviews were held with six KIs who were in‐charge of maternity units of health facilities. Latent content analysis technique was used for data analysis.ResultsTwo main themes emerged; ‘feeling exposed and powerless’, and ‘seeking safety and empathy’. The categories identified in the first theme were “the dilemma of becoming an adolescent mother” and “lack of decision power”. In the second theme the following categories were identified: “cultural practices and beliefs about birth”, “expectations and experiences”, “transport, a key determinant to health seeking”, and “dealing with constraints”. Adolescents felt exposed and powerless due to the dilemma of early motherhood and lack of decision making power. The adolescent mothers seemed to be in continuous quest for safety and empathy. In so doing they are part of cultural practices and beliefs about birth. They had expectations about the health care services but their experiences of the services were rather negative. Transport was a key determinant for health seeking and adolescents to some extent had learnt how to cope with constraints they face.Conclusion and implicationsPregnant adolescents seek health care in both modern and traditional health sectors in order to get safety and empathy. However, our findings indicate that they mostly utilize the traditional sector because it is most accessible in terms of distance, cost and cultural context. Adolescent mothers are disempowered in decision making because of their pregnancy state which often puts them in dilemma. We therefore suggest that policy makers need to improve health systems (including the traditional sector) especially maternal health services for adolescent girls. Improved infrastructure and attitudes of health worker as well as training in delivery of adolescent health services is critical.

Pregnancy and childbearing are well recognized as having significant, long-term consequences for teenagers. Recent literature documents an array of negative outcomes for children born to adolescents, with a range of factors identified as contributing to the problems observed in these children. These include (1) the characteristics of those teenagers most likely to become parents, (2) the social and economic consequences of early childbearing, (3) the increased biologic vulnerability of children born to teenagers, and (4) the nature of parenting by teenagers. It has been acknowledged that adolescent parents tend to come from high-risk families, have poor academic achievement, and live in our most disadvantaged communities and therefore, biologic, economic, and behavioral factors contribute to the increased likelihood of teenagers having children who are vulnerable to physical and developmental problems. Teenaged parents face many obstacles to economic and social success, and these further influence the environment in which their children grow up. Adolescents also experience many difficulties in adjusting to parenthood and display a range of suboptimal parenting practices. Whereas some of these factors appear highly resistant to change, others have clear clinical, programmatic, and policy implications.
