Background: Management of inguinal hernia by minimally invasive incision and minimal dissection with optimal outcomes remains an unmet need for general surgeons. The most common hernia repair procedures are Lichtenstein hernioplasty and laparoscopic hernia repair; TAPP and TEP. The modification of mesh plug repair (MPR); one inch incision-MPR (OI-MPR); could achieve optimal outcomes comparable to that of laparoscopic hernia repair. The objective of the study was to assess OI-MPR approach for management of primary indirect inguinal hernia and its outcomes in comparison to laparoscopic TAPP repair.Methods: During the period from September 2017 to March 2019, 63 patients with indirect inguinal hernia were selected randomly to one of two groups; group-A included 35 patients underwent one inch incision-mesh plug repair (OI-MPR), group-B included 28 patients underwent laparoscopic TAPP.Results: The mean operative time of group-A was 31.74±4.74 min which was significantly shorter than that of group-B (81.22±7.44 min). The mean hospital stay duration in group-A was 0.57±0.18 day and in group-B, it was 1.18±0.39 day. The patients of group-A returned to normal physical activities earlier than the patients of group-B. The duration of postoperative pain was shorter and less severe in group-A than that of group-B. Regarding complications, there was no statistically significant difference found between both groups.Conclusions: One inch incision-MPR is a simple, feasible and cost-effective approach with low morbidities and low burden on the patients and the healthcare facilities.

ach year, more than 20 million procedures to repair inguinal hernia are performed worldwide. 1 The lifetime risk of developing an inguinal hernia is 27% for men compared with only 3% for women. 2 However, studies have reported that 8% to 16% of patients with inguinal hernia are females. 3,4 Nevertheless, to our knowledge, no randomized controlled trial (RCT) or systematic review has specifically addressed repair of groin hernia in women. 5 Guidelines for repair of inguinal hernias in women are based mainly on subgroup analyses of national database studies in which most patients were men. [5][6][7] Differences regarding pelvic and inguinal anatomy between men and women 8 could mean that the recurrence rate of inguinal hernias may differ, and recurrences after repair of inguinal hernia in women may be a previously undiagnosed femoral hernia. 9,10 Therefore, the rate and type of recurrence for in-guinal hernias needs to be assessed specifically in women. This study investigates rates and types of recurrences in women who underwent repair of a primary inguinal hernia. MethodsThis study was reported according to the Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) guideline. 11 A detailed protocol was registered at the PROSPERO systematic review registration site (registration CRD42017081152). 12 Participants in the study were women age 18 years or older who had undergone repair of a primary unilateral or bilateral inguinal hernia. Studies including participants with solely recurrent inguinal hernias or solely femor-IMPORTANCE To our knowledge, a systematic review has not yet been performed that specifically addresses the management of inguinal hernia in women. Recurrence after repair of inguinal hernia is not unusual in women and may be a previously undiagnosed femoral hernia, which is rarely seen in men. OBJECTIVE To investigate rates and types of recurrences in women who had undergone repair of primary inguinal hernia.EVIDENCE REVIEW PubMed, Embase, and the Cochrane databases were searched in September 2017 for studies reporting recurrences after repair of primary inguinal hernia in women. Crude rates of recurrence (number of recurrences/number of women) were calculated after open and laparoscopic repairs, and the type of recurrent hernia was registered, if noted in the studies. This review is reported according to the PRISMA guideline. FINDINGS A total of 55 studies were included, comprising 43 870 women (mean age, 42-69 years; median age, 57 years). Five studies were randomized clinical trials, 14 were prospective cohort studies, 7 were prospective database studies, and 29 were retrospective cohort studies. Twenty studies reported recurrence after laparoscopic repair, with a crude recurrence rate of 1.2% (27 of 2257) (range, 0%-5%) and a median follow-up of 24 months. Simulated training model in a low cost for laparoscopic inguinal hernioplasty

Purpose: Develop a 3D model for the simulation of laparoscopic inguinal hernioplasty transabdominal preperitoneal (TAPP). Methods: This is an experimental study, 18 participants were selected, divided into three groups, experimental (GE) surgeons in training, control (GC) experienced surgeons and Shaw (GS) nonexperienced surgeons. The simulation in the 3D model was carried out in 6 sessions fulfilling the 5 stages. Opening the peritoneum with the creation of the preperitoneal space; identification of important structures; hernia identification and reduction; placement and fixation of the mesh in Cooper's ligament and closure of the peritoneum. Results: In the 1st stage, the GE obtained an average of 1.25 ± 0.42 in the 1st session and 3.25 ± 0.62 in the 6th session (p = 0.05) and in the 5th stage 0.91 ± 0.29 in the first session. 1st session and 1.91 ± 0.29 in the 6th session (p = 0.001), with no significant difference between groups. The learning and skill curve in the SG represented 1.08 ± 0.29 1st and 3.50 ± 0.90 6th session (p = 0.001). Conclusion: The creation of a systematization of training in simulation applied to the three-dimensional model enabled gain in laparoscopic skills and underpinned its theoretical and practical foundations.

Abstract: The recurrence and wound infection rates were similar after hernia repair using the PHS and MP methods. Patients undergoing PHS repair developed fewer subcutaneous hematomas. An older age (≥65 years) was a significant independent risk factor for recurrence.
