Shoshin beriberi: an underdiagnosed condition?

Abstract: Shoshin beriberi, a fulminant form of heart failure due to thiamine deficiency has a different presentation to the classical form of beriberi heart failure. It is characterized by a cold periphery, low blood pressure, renal shutdown and a severe metabolic acidosis. The true incidence is unknown. Two patients were seen within a few months in a general hospital and in both dietary deficiency of thiamine was a major factor.

“…From a clinical perspective, the I-Lan outbreak illustrates the importance of considering vitamin deficiency in the differential diagnosis of illness, especially among patients whose diets are very restricted or unusual, such as alcoholics, patients reliant on total parenteral nutrition, or those on very severe weight loss diets. 5,6,7,26 Unless beriberi is considered in the differential diagnosis of neuropathy, high output cardiac failure, lactic acidosis, or shock, an easily treated, potentially fatal condition may be overlooked.…”

“…In its most fulminant form, this condition is characterized by hypotension, tachycardia and lactic acidosis. If untreated, patients die within hours from circulatory collapse and pulmonary edema [3]. This condition often goes unrecognized since it is easily confused with other illnesses.…”

“…Owing to its short storage cycle, marginal thiamine deficiency can occur as early as 10 days after deprivation (Finglas, 1993). Symptoms of beriberi include poor appetite, fatigue, peripheral neuritis and in some cases edema and cardiac failure (Engbers, et al, 1984;Pang et al, 1986). Severe thiamine deficiency is linked to two classical conditions: beriberi and Wernicke-Korsakoff syndrome.…”
