Should the dose of levothyroxine be changed in hypothyroidism patients fasting during Ramadan?

Abstract: Background/aim: Muslims worship by fasting from predawn (suhoor) until sunset (iftar) for 30 days in the religious month of Ramadan. In addition to prolonged hunger, patients fasting with a diagnosis of hypothyroidism take their doses of levothyroxine (LT4) outside of daytime fasting hours. The purpose of our study is to compare the values of hypothyroid patients which have been obtained through thyroid function tests before and after Ramadan.Materials and methods: Ninety-seven patients; ranging from 18 to 65 … Show more

“…Thyroxine absorption can be affected and lead to suboptimal control of hypothyroidism [33]. It has been proposed that the best time to take thyroxine is later at night provided that no additional heavy food is eaten between Iftar and the late night hours [34]. These recommendations have used Level 3 evidence and a Grade D by the IDF DAR writing group.…”

“…Furthermore, Or Koca et al investigated whether the dose of levothyroxine was changed during RF in 97 patients with a wide age range on a stable dose of LT4 for at least 6 months. 22 They demonstrated a significant increase in serum TSH levels after Ramadan but no significant change in serum-free thyroxine (fT4) levels. They suggested making a small increase in LT4 dose before Ramadan in some hypothyroid patients wishing to fast.…”

“…Therapeutically, small increases in levothyroxine dose by 25–50 μg/day from the beginning of Ramadan till 2–3 weeks post-Ramadan have been previously suggested to improve the control of hypothyroidism ( 11–16 ). However, the effect of this small increase in levothyroxine dose or the timing of its administration during Ramadan has not been evaluated in a prospective trial.…”
