Significance of mild cytologic atypia in women infected with human immunodeficiency virus

Abstract: Mild cytologic atypia, a frequent diagnosis on Papanicolaou smears from HIV-infected women, is associated with CIN. We recommend that all HIV-infected women with mild cytologic atypia be referred for colposcopy.

“…It is noteworthy that the prevalence of LSILs increased slightly with the clinical stage of HIV-related disease, as reported elsewhere, 11 and sharply with a decrease in CD4 cell count in HIV-1-infected women, in agreement with other studies. 13,14,25,26 In contrast, the degree of immunosuppression did not influence the risk of HSILs in HIV-1-infected women. Our study shows that women with cervical dysplasia were not very immunosuppressed: SILs occurred at an early stage of HIV disease.…”

“…2 Cigarette smoking was very rare in our study and unrelated to cervical dysplasia. 25,36,40 In contrast, chewing tobacco use was associated with HSILs and was more frequent among women with ICC. Even though chewing tobacco use was rare, found mostly among women of low educational level, public health authorities must be made aware of the benefits to women's health that would result from preventing addiction to chewing tobacco in Africa.…”

“…If misclassification of cervical lesions was nondifferential with respect to HIV status, our observed OR would most likely be an underestimate of the true relative risk. However, it may be that the observed rates of cytologic abnormalities reported among HIV-positive women underestimate the true prevalence of cervical disease, because the performance of Pap smears may be unusually poor in HIV-positive women [62,63]. However, if high-grade lesions were, indeed, more likely to be missed in our HIV-positive women, then our observed association likely underestimates the true risk of having developed HSILs or ICC associated with HIV-1 or HIV-2 infection.…”
