Background: With the rapid change of lifestyle, the incidence of coronary artery disease (CAD) was gradually rising among young people, but the specific reason for it was unclear. There were many evidences suggesting free fatty acids (FFAs) were involved in progression of CAD, our study was designed to discuss the relationship between serum FFAs and acute coronary syndrome (ACS) and the complexity of coronary artery disease in premature coronary artery disease (pCAD) patients. Methods: 502 pCAD patients suffering from ACS confirmed by coronary angiography in Beijing Anzhen Hospital from January to December 2019 were enrolled (ACS group), and 141 patients of the same age whose coronary angiography result was negative were selected during the same period (control group). The clinical data and characteristics of CAD among two groups were gathered and compared.Results: The level of FFAs in ACS group was significantly higher than control group (P < 0.001), and the value of FFAs in AMI group was higher than UA (P < 0.05) in both genders. The concentration of FFAs in three-vessel disease group was higher than double-vessel disease group and double-vessel disease group was higher than single-vessel disease group. The linear correlation coefficient between FFAs value and SYNTAX score was 0.370, P < 0.001. The AUC of FFAs to diagnose ACS was 0.798 (0.743-0.853, P < 0.001) in males, and 0.738 (0.677-0.800, P < 0.001) in females. Logistic regression analysis showed that elevated level of FFAs [odds ratio (OR) = 7.06, 95% confidence interval (CI) 4.40-11.33, P < 0.001] was independently associated with ACS. Conclusions: Concentration of FFAs is an independent risk factor associated with the incidence of ACS in pCAD patients and positively correlated with the complexity of CAD.

• Background• It is not currently known whether free fatty acids can be used as predictors for the prognosis of patients with acute ST-segment elevation myocardial infarction. The results of many studies on the correlation between free fatty acids and the prognosis of patients with acute myocardial infarction are inconsistent. The purpose of this clinical study is to determine the predictive effect of different free fatty acids in the plasma on cardiovascular events within the 12 months after a patient suffers an acute ST-segment elevation myocardial infarction.• Methods• This study is a prospective, observational, single-center cohort study. After enrollment, all participants will receive standard treatment and will be tested for 31 kinds of plasma free fatty acids. The patients will be followed up 2, 6, and 12 months after discharge. The primary endpoint is the compound cardiovascular events from onset to 12 months, including the cumulative incidence of all-cause death, heart failure, or coronary revascularization events. According to the power and sample size calculation, it will be necessary to enroll 747 patients in this study. After follow-up, data will be collected and analyzed to explore the predictive effect of different free fatty acids in plasma on the prognosis of acute elevated ST-segment myocardial infarction, and the risk factors of the endpoint events within 12 months.• Discussion• This study will provide new insightful evidence of serum indicators indicating the 12-month cardiovascular events of acute ST-segment elevation myocardial infarction.Trial registration: This study was retrospectively registrated at Chinese Clinical Trail Registry (https://www.chictr.org.cn/) on 14th July,2021. BACKGROUND Sulfonylurea (SU) is a commonly used antidiabetic drugs effective for type 2 diabetes mellitus. Previous studies have reported that the SU treatment could alter the serum free fatty acid (FFA) concentration in diabetic patients; however, their exact effects remain unknown. AIM To assess the impact of SU on the FFA level in diabetic patients. METHODS A systematic literature search was conducted by consulting the PubMed, EMBASE, Cochrane Library, Reference Citation Analysis ( ), and Web of Science databases from January 1, 1991 to July 30, 2021. Either a fixed-effects model or random-effects model was applied to study the association between SU treatment and FFA concentration according to the heterogeneity test. Two investigators independently performed data extraction. The mean difference (MD) and corresponding 95% confidence interval (CI) were used to measure effect size. R3.5.1 software was utilized for conducting statistical analyses. RESULTS A total of 13 studies with 2273 individuals were selected. Results indicated that FFA concentration increased slightly after treatment with SU (MD = 0.08, 95%CI: 0.03-0.12, P < 0.01). In addition, we found that SU treatment combined with other antidiabetics could also increase the concentration of serum FFA (MD = 0.14, 95%CI: 0.01-0.28, P < 0.01). Regarding the type of SU, there was no significant difference in FFA concentration with glimepiride or glibenclamide. FFA concentration was higher at ≥ 12 wk (MD = 0.09, 95%CI: 0.04-0.13) but not at < 12 wk (MD = 0.01, 95%CI: -0.07-0.09). CONCLUSION SU treatment could increase the serum FFA concentration in diabetic patients. The fundamental underlying mechanism still needs further investigation.
