Simple Arm Tourniquet as an Adjunct to Double-Cuff Tourniquet in Intravenous Regional Anesthesia

Abstract: Background:Intravenous Regional Anesthesia (IVRA) is a well-known technique for producing analgesia during surgical procedures in the extremities. However, the rapid onset of pain following the deflation of a double-cuff tourniquet during IVRA is a serious disadvantage, leading patient suffering. Objectives:The aim of this study was to evaluate the clinical effectiveness of a pneumatic arm tourniquet applied 2 cm above the double-cuff tourniquet in controlling the pain that occurs after its deflation.Patients a… Show more

“…Local anesthesia is a safe and effective method in different surgeries such as upper extremity surgeries ( 1 ). This approach has several advantages over general anesthesia including lack of pain during operation, decreased postoperative pain and analgesic dose, more hemodynamic stability, and increased patients satisfaction ( 2 - 4 ). Supraclavicular brachial plexus block is a common method in forearm surgeries ( 5 ).…”

“…Intravenous regional anesthesia (IVRA) is a simple type of regional anesthesia that has many advantages such as: reliability, effectiveness, economic for day care, and safe for emergency surgery when patient is full stomach ( 1 - 4 ). However, it has some limitations such as lack of postoperative analgesia, tourniquet pain, insufficient muscle relaxation, and local anesthetic toxicity ( 5 , 6 ).…”

“…In the study of Jafarian et al ( 19 ), a modified Bier's block method versus the traditional one performed on patients undergoing surgery of upper limb bones showed considerable pain relief at surgical and tourniquet sites during the operation until one hour thereafter. In another study by Jafarian et al ( 20 ), pneumatic arm tourniquet as an adjunct to intravenous regional anesthesia (IVRA) provided acceptable analgesia, especially in the elderly, although it needs to be more considered in future studies. However, it should be noted that it is technically more difficult and time-consuming for emergencies.…”
