Simple prognostic model to predict survival in patients with undifferentiated carcinoma of unknown primary site.

Abstract: Good-prognosis patients had a performance score of 0 (World Health Organization [WHO]) and an alkaline phosphatase serum level less than 1.25 times the upper limit of normal (N). These patients had a median survival duration greater than 4 years. Intermediate-prognosis patients were characterized by either a WHO performance status < or = 1 or an alkaline phosphatase level > or = 1.25 N. These patients had a median survival duration of 10 months and a 4-year survival rate of only 15%. The poor-prognosis group h… Show more

“…Three subgroups were identified with median survivals of more than 4 years, 10 months and 4 months according to the presence of zero, one or two adverse prognosticators, respectively. However, this model was limited to CUP patients with poorly differentiated adenocarcinoma or undifferentiated carcinomas and was not validated in an independent data set of patients [11]. Investigators at the M.D.…”

“…Except for subgroups of patients with clinical and pathologic features requiring specific guidelines that may translate into prolonged survival, the benefit of chemotherapy remains questionable, although such a benefit is suggested by historical comparisons [7]. Previous studies dealing with prognostic features in patients with CUP identified a number of independent adverse variables such as male sex, a poor performance status, a high number of metastatic sites, the presence of liver metastases, elevated levels of serum alkaline phosphatase and lactate dehydrogenase [8][9][10][11][12] (Table 4). Using performance status and serum alkaline phosphatase level as independent prognostic factors, Van der Gaast et al built a prognostic model in a population of 79 patients.…”

“…The variables we considered in our analysis are those that, especially in the last 2 decades, have been suggested by various authors as carrying prognostic value [3,8,9,10, 13, 14, 16, 41]. However, there is little agreement between different reports.…”
