Simplified and Strong

Abstract: Surgical treatment of thumb basal joint arthritis is aimed at pain relief, restoration of pinch strength, and stability. We describe a variation of the abductor pollicus longus (APL) suspension arthroplasty originally described by Thompson, which maximizes strength of repair and is technically simple to perform. The technique consists of a trapeziectomy followed by resection of a slip of the APL and secure docking of the tendon into the base of the index metacarpal base. Stability is enhanced with a 3.0 mm bio… Show more

“…Third, tendon harvest can be done from the same incision, whereas using FCR may require additional forearm incisions. Julien et al. (2013) described a modification of the APL suspensionplasty without interosseous drilling through the first metacarpal with tendon fixation using a biotenodesis screw into the base of the index metacarpal.…”

Julien et al (2013) described a modification of the APL suspensionplasty without interosseous drilling through the first metacarpal with tendon fixation using a biotenodesis screw into the base of the index metacarpal. This was performed on 50 patients, and follow-up at 17 months reported improved functional outcomes, decreased pain and fewer complications than the original APL suspensionplasty with interosseous drilling (Nanno et al, 2019).…”

“…Sufficient fixation force and clinical results have already been reported with tenodesis screw fixation to the second metacarpal of the APL. 23,24 Therefore, tenodesis screw fixation was performed from the second metacarpal entry of the APL tendon while maintaining maximum tension on the APL tendon with the thumb in full palmar and radial abduction (Figure 2). Subsequently, we moved on to fixation of the first and second metacarpals using suture button device, the Mini TightRope (Arthrex, Inc.).…”

“…3,4 The interference screw has been successfully utilized as a method of fixation in the upper and lower extremities. 5,6 The use of an interference screw eliminates the need for multiple assistants in the classic ligament reconstruction and tendon interposition (LRTI) procedure described by Burton and Pellegrini. 7 Previous descriptions of this operation have included a dorsal midline incision and dorsoradial incision with excellent outcomes; however, high failure rates have been reported in hand surgeons with <12 years' experience.…”
