Simulation and Web-based learning increases utilization of Bier block for forearm fracture reduction in the pediatric emergency department

Abstract: Objectives: Bier block (BB) is a safe and effective alternative to procedural sedation for analgesia during forearm fracture reductions, yet remains infrequently used in the pediatric emergency department (PED). No standardized methods of BB training have previously been described. The objective of this study was to determine whether a multimodal instructional course increases comfort with BB and translates to increased use of this technique. Methods: A novel interdisciplinary simulation and Web-based training… Show more

“…88 Other popular modalities included full-body mannequins, 89 part-task mannequin trainers, 90 and hybrid models. 85 The use of computer/web-based simulations, 91 virtual reality, 92 and tissue models 93 was less frequently reported. Most studies described the inclusion of features related to simulation effectiveness, such as individual or team debriefing, opportunities for repeated practice, and standardization across participants.…”

“…User-perspective surveys will be developed and reported according to published guidelines [9]. Specific data on app utilization and timing of use will be tracked among users in our PED in parallel to real-world clinical use of BB for pediatric fracture reductions [6]. Tracking of app usage in our PED will make use of the external tester function of Apple TestFlight software.…”

“…Several recent researches [5][6][7][8] in the last twenty years, have lifted the lid from different problems in healthcare systems and focused on providing solutions to those issues, specifically in EDs which they suffer from several phenomena (overcrowding, resources lack, etc.). Different methodologies and techniques in the literature have applied in the process of modeling and optimization of ‗ED' systems.…”
