Simulation-Based Training in Brain Death Determination

Abstract: Baseline knowledge of brain death determination among providers was low but improved greatly after the course. Our intervention represents an effective model that can be replicated at other institutions to train clinicians in the determination of brain death according to evidence-based guidelines.

“…20 These results were mirrored in a test on DNC given to practicing neurologists, who were able to correctly answer only 54% of questions. 21 It is important for medical students to receive a solid education on this topic and for neurologists and other clinicians who are responsible for declaration of DNC to receive continuing education throughout their careers. Our findings suggest that this education should be focused on both the process of declaration of DNC and the ideal way to handle families who request continuation of organ support after DNC.…”

“…18,20 Another barrier to performance of accurate determinations is the fact that qualifications for examiners and physician awareness of the intricacies of brain death determination vary. 18,21 Frequent education and reeducation is warranted. This can be accomplished through simulation sessions.…”

“…Important parts of the clinical exam, such as the gag reflex, were not defined in six countries and the apnea test details in four. The number of clinical exams varied from 1 to 3 and the time interval between exams from no minimum to 12 h. The list can go on, but the reader, like the authors, can easily conclude one of two things: that an international consensus on BD determination is imperative and that some clinical competency requirement [5] or check lists (like those used in the operating room or the one provided by the AAN [2]) may put further order into an already chaotic situation. One step in the right direction is the paper by MacDougall et al [6]. The authors describe a BD didactic course and simulation exercise that they developed at Yale.…”
