Simultaneous defecography and peritoneography in defecation disorders

Abstract: Simultaneous defecography and peritoneography can be performed without technical difficulties or complications. Peritoneal outlines and pouches can, therefore, be studied directly during the act of defecation. An unexplained widening of the rectovaginal space at defecography can be clarified as a peritoneocele, with or without an enterocele. Peritoneocele can be of three different types: rectal, septal, or vaginal.

“…Impaired defecation is well known in rectocele patients [1,6] but less recognized in patients with an enterocele. It is known that anterior rectal wall procidentia may be caused by an enterocele [3,7,8,9], but the impact on rectal evacuation is not fully understood. Wallden [3] has described a defecation block due to an enterocele, …”

“…Rectocele was de¢ned as a bulging of the anterior rectal wall into the lumen of the vagina exceeding 2 cm in size [Shorvon et al, 1989].The size was measured from an extended line of the anal canal to the tip of the rectocele [Yoshioka et al, 1991]. Abnormal pelvic £oor descent was de¢ned as a lowering of the perineal outline exceeding 2 cm at straining [Bremmer et al, 1995].…”

“…All patients underwent defecoperitoneography as described by Bremmer et al [1995]. Rectocele was de¢ned as a bulging of the anterior rectal wall into the lumen of the vagina exceeding 2 cm in size [Shorvon et al, 1989].The size was measured from an extended line of the anal canal to the tip of the rectocele [Yoshioka et al, 1991].…”

“…A simultaneous rectovaginal examination determined size and location of a rectocele and enterocele (bowel palpable between vagina and rectum). Enteroceles bulging into the anal canal on digital examination and proctoscopy were classified as internal anterior rectal wall procidentia (rectal type of an enterocele or peritoneocele according to Bremmer et al, 1995). If the enterocele protruded through the anal canal, an external anterior rectal wall prolapse was noted.…”
