Simultaneous Femoral and Tibial Osteotomies During Total Knee Arthroplasty for Severe Extra-Articular Deformity

Abstract: Total knee arthroplasty (TKA) performed in knees with mild or moderate intraarticular deformity often can be resolved with careful ligament balancing and bone resection. However, extra-articular deformity may require an osteotomy to safely create rectangular flexion and extension gap balance. In these challenging situations, restoring the mechanical axis through intra-articular bone resection and soft tissue releases alone can lead to excessive bone loss and ligament instability. We report a case of TKA with c… Show more

“…The osteotomy can be stabilized in different ways [5,9,10,14,19], but stems are our first choice. In these cases it is important to have modular prosthesis that allow to choose the stem that better fills the medulary canal, in order to optimize the post-surgical stability of the osteotomy.…”

“…There is a wide range of opinions regarding which is the most appropriate way to correct the axis of a limb with extra-articular deformity when performing a TKA [1,3,[7][8][9][10][11]. However, there is certain consensus in the fact that if the femoral deformity in the coronal or sagital plane is over 20° or if the tibial deformity exceeds 30°, the correction performed only by means of intraarticular "unusual" bone cuts and the consequent release of soft tissues might generate a complex instability.…”

“…Some articles reported aetiology of EAD due to malunited osteotomy. Relative less number of cases reported with Paget disease [6) Hypophosphatemic rickets [7] , Congenital dysplasia [8] , Postpolio sequelae [9,10] . While performing TKA in a knee with complex deformity the surgeon must understand the quantity and quality of the deformity, which exerts direct and indirect impact over the joint.…”

“…We advise in all cases the use of a long tibial stem to bridge the osteotomy site and gain a measure of distal fixation [18]. Vielpeau et al [19], in a study of 14 patients treated by combined osteotomy and long-stemmed TKA without additional fixation, reported a normal mechanical axis in 70 %, a mean KSS of 90.2 and a global score of 170.…”
