Simultaneous magnetic resonance imaging of ventilation distribution and gas uptake in the human lung using hyperpolarized xenon-129

Abstract: Despite a myriad of technical advances in medical imaging, as well as the growing need to address the global impact of pulmonary diseases, such as asthma and chronic obstructive pulmonary disease, on health and quality of life, it remains challenging to obtain in vivo regional depiction and quantification of the most basic physiological functions of the lung-gas delivery to the airspaces and gas uptake by the lung parenchyma and blood-in a manner suitable for routine application in humans. We report a method b… Show more

“…Because airway structures will limit the diffusion of gas, measurement of the atomic motion will provide information about the lung microstructure. Both gases have been used in humans, large animals, and, more recently, small animals in both healthy and diseased conditions and have provided important insights into the three-dimensional structure of the lungs (64)(65)(66)(67)(68)(69)(70)(71)(72)(73). The usefulness of this approach to treatment of human disease is obvious, with greater ability to determine the changes in lung microstructure as a function of disease and over time.…”

“…Unfortunately, dissemination of 3 He MR imaging faces challenges given the constrained supply of 3 He and recent increased 3 He use for homeland security applications, which has driven up costs from approximately $100 per liter to approximately $500 per liter ( 14 ). Therefore, it is now recognized that development of imaging with the more readily abundant isotope 129 Xe should be pursued ( 15,16 ).…”

“…In hyperpolarized xenon MRI studies, prior hyperpolarization of the gas through a process called spin-exchange optical pumping (SEOP) is the necessary step to make the gas MRvisible (22). After hyperpolarization, the gas can be inhaled by the subject or by the animal to visualize lung ventilation function (23), xenon gas exchange (23,24), or other distal organs of interest (25). After inhalation the gas diffuses from the lung airspaces to the lung parenchyma and to the blood.…”
