Simultaneous Open Reduction and Salter Innominate Osteotomy for Developmental Dysplasia of the Hip

Abstract: We have studied retrospectively 37 hips in 36 children at an average of 91 months after simultaneous open reduction and Salter innominate osteotomy for developmental hip dysplasia. At the latest review 97.3% were clinically and 83.8% radiologically good or excellent. In three hips (8%) there were signs of avascular necrosis, but only one had been symptomatic. There were no cases of recurrent posterior displacement.

“…In the literature anterior open reduction has been used in isolation in this age group but has also been combined with pelvic osteotomy [7,14,22,28,30,32,40,44,46], proximal femoral osteotomy [28,31,38,46], or both in a single stage [3,10,11,26,31,39]. It is unclear which surgical procedure or combination of procedures is least likely to result in clinically relevant AVN (K&M Types II-IV or equivalent) and most likely to provide satisfactory radiological and clinical results (Severin I or II or equivalent and McKay excellent or good rating or equivalent, respectively), while minimizing the requirement for further surgical intervention.…”

“…During the last 36 years, the anterior approach has been reported less frequently [1,10,17,18,20,23,25,30,31,47,58,70,71,73,79,80] than the medial approach [4, 6, 7, 11, 16, 21, 22, 33, 36, 37, 39, 40, 46, 48-50, 52-54, 60, 69, 77, 78, 85, 88], especially in younger children.…”
