The synthesis of evidence from trials and medical studies using meta-analysis is essential for Evidence Based Medicine. However, problematical outlying results often occur even under the random-effects model. We propose a model that allows a long-tailed distribution for the random effect, which removes the necessity for an arbitrary decision to include or exclude outliers. In this approach, they are included, but with a reduced weight. We also introduce a modification of the forest plot to show the downweighting of outliers. We illustrate the methodology and its usefulness by carrying out both frequentist and Bayesian meta-analyses using data sets from the Cochrane Collaboration.

Nonsteroidal anti-inflammatory drugs (NSAIDs) are the mainstay of therapy for the management of acute pain. Cyclooxygenase (COX) enzyme is of particular interest because it is the major target of NSAIDs. Although NSAIDs are remarkably effective in the management of pain and inflammation, their use is limited by several adverse effects including gastrointestinal bleeding and ulceration, impaired renal function, and inhibition of platelet aggregation. Discovery of a second cyclooxygenase, COX-2, led to the hypothesis that NSAID side effects could be decreased, as the inhibition of COX-2 is more directly implicated in ameliorating inflammation while the inhibition of COX-1 is related to adverse effects in the GI tract. This stimulated the development of selective COX-2 inhibitors (coxibs) that are better tolerated than nonselective NSAIDs but comparable in analgesic efficacy. This article provides an overview on the therapeutic use of selective COX-2 inhibitors for relief of acute pain, largely based on clinical trials in patients undergoing the surgical removal of impacted third molars, with focus on analgesic efficacy and the potential safety associated with their use compared to dual COX-1/COX-2 inhibitors.

Paraoxonase was purified from human serum using sepharose-4B-L-tyrosine-1-napthylamine affinity chromatography. This enzyme was purified 1797-fold. SDSpolyacrylamide electrophoresis of the enzyme indicates a single protein staining band with an apparent Mr of 43 kDa. The kinetic properties of the purified enzyme were determined. The V max and K m are 231.5 µ/mg protein (EU) and 3.96 mM using substrate paraoxon respectively. The effect of acetylsalicylic acid on purified human serum paraoxonase has been investigated in vitro. The inhibition and activation effects of acetylsalicylic acid on serum paraoxonase activity were measured spectrophotometrically using paraoxon as the substrate. The phenotypic distribution of paraoxonase was determined using the dual substrate paraoxon and phenyl acetate as substrates. It was observed that acetylsalicylic acid inhibited human serum paraoxonase activity. In addition, an in vivo study was performed for acetylsalicylic acid in Sprague-Dawley rats. It was demonstrated that paraoxonase in the serum of SpragueDawley rat was activated, but activity in liver and heart was significantly inhibited.
