Single-port cholecystectomy in a patient with situs inversus totalis presenting with cholelithiasis: a case report

Abstract: IntroductionSitus inversus totalis (mirror image organs) is a rare condition and may affect the intra-abdominal viscera as well as the intrathoracic organs. Cholelithiasis is not more common in these conditions, but the diagnosis may be more difficult.Case presentationWe present the case of a 59-year-old African woman with gallstones and situs inversus totalis. A single-port cholecystectomy was performed using a single trocar access device (SITRACC).ConclusionsThe procedure was uneventful, showing that this ap… Show more

“…There have been isolated reports of situs inversus associated with peptic conditions [14], ulcer perforation [15], amoebic liver abscess [8], acute cholecystitis [7,16], cholelithiasis [6,17,18], acute appendicitis [4,19], and intestinal obstruction [20]. The occurrence of carcinoma of the gastric cardia with situs inversus is very rare, and very few cases have been reported in the literature [5,21-24].…”

“…5 While there are several reports of single-incision or single-port cholecystectomies in patients with SIT, the present case report appears to be the first in the medical literature of a single-incision appendectomy in a patient with SIT, although the technique has been mentioned previously. [12][13][14][15][16][17] The single-incision procedure described in the current case report took only a few minutes longer than a standard multi-port appendectomy. Ultrasonic shears are not normally used for appendectomies as they increase the cost of the procedure; 11,18 however, in the present case, they were deemed suitable as alligator needle/grasper forceps would not be able to coagulate the tissues if mesoappendix bleeding occurred with cautery.…”

“…Other laparoscopic techniques for single site cholecystectomy are reported as safe and effective approach. [2][3][4][5][6][7] However, these all require advanced surgical skills and experiences to perform a safe laparoscopic cholecystectomy. Recently, single site robotic surgical system has been introduced to make laparoscopic surgery easier, safer, and more effective.…”

“…However, technical diffi culties and limited instrumental movements resulting from crowdedness of instruments through one single incision make surgeons require specialized trocars, instruments, and skillful surgical techniques. [2][3][4][5][6][7] We have previously introduced the technique, named "single fulcrum-laparoscopic cholecystectomy (SFLC)", a modified version of single incision. The advantage of SFLC is that it does not need any specialized ports or instruments.…”
