Single-port laparoscopic cholecystectomy in situs inversus totalis using the E.K. glove port

Abstract: Laparoscopic cholecystectomy has become the standard treatment for symptomatic cholelithiasis in patients with situs inversus totalis (SIT). In an effort to reduce morbidity and improve the cosmesis single-port laparoscopic cholecystectomy has recently emerged, where the surgery is done through a single port, typically the patient's navel. This improves the cosmesis, lessens post-operative pain and ensures virtually a “scar less” surgery. We report a case of successful single-port laparoscopic cholecystectomy … Show more

“…5 While there are several reports of single-incision or single-port cholecystectomies in patients with SIT, the present case report appears to be the first in the medical literature of a single-incision appendectomy in a patient with SIT, although the technique has been mentioned previously. [12][13][14][15][16][17] The single-incision procedure described in the current case report took only a few minutes longer than a standard multi-port appendectomy. Ultrasonic shears are not normally used for appendectomies as they increase the cost of the procedure; 11,18 however, in the present case, they were deemed suitable as alligator needle/grasper forceps would not be able to coagulate the tissues if mesoappendix bleeding occurred with cautery.…”

“…Different access ports have been used in several successful operations. 10 , 11 In addition, the positions of the surgical instruments can be adjusted to facilitate operations in patients with SIT. 12 However, this has been quite troublesome for other operations in patients with a normal anatomy.…”

“…Since de Campos Martins et al first reported a successful laparoscopic cholecystectomy in a patient with SIT in 1991, there have been 57 reports of laparoscopic cholecystectomy in patients with SIT (2). This procedure can be performed safely in SIT patients, but only five cases have been published in English on single-incision or single-port laparoscopic surgery (3)(4)(5)(6).…”
