Sinusitis in Pilots of Different Aircraft Types: Is Allergic Rhinitis a Predisposing Factor?

Abstract: Despite careful selection, allergic rhinitis is still a very common disease in pilots and may pose a risk of acute sinusitis. The lower prevalence of acute sinusitis in combat than in transport pilots with rhinitis may be explained by vasoconstriction due to psychological and physiological stress during flight missions.

“…Estimates of prevalence range from 34% in divers, 13 to 19.5–25% in pilots. 14 , 15 Concurrent sinonasal inflammation ( e.g. , allergic rhinitis) further increases the prevalence of barosinusitis in pilots, with rates reported as high as 55% in commercial pilots and 34% in high-performance fighter pilots.…”

“…A study on morbidity among pilots from New Zealand reported that pilots have a lower prevalence of asthma (4.2%) as compared to the general population (9.6%) in New Zealand [ 10 ]. Allergic rhinitis is a common disease among pilots, which can increase the risk for acute sinusitis [ 11 ]. Air recirculation has been suggested as a risk factor for airway infections in aircraft, but one study found the same prevalence of airway infections among pilots on flight with cabin air re-circulation (10.6%) and without cabin air re-circulation (10.1%) [ 6 ].…”
