Size and Shape of Associations of OGTT as Well as Mediating Effects on Adverse Pregnancy Outcomes Among Women With Gestational Diabetes Mellitus: Population-Based Study From Southern Han Chinese

Abstract: Conclusion: Higher fasting plasma glucose was more strongly linked to adverse pregnancy outcomes among GDM patients. Both linearity and Non-linearity of associations between glucose and complications should be taken into account. A careful reconsideration of GDM with hierarchical and individualized management according to OGTT is needed.

“…In the present study, the association between HbA1c and the risk of primary C-section varied in different pre-pregnancy BMI groups and maternal age groups. Studies have revealed the utility of HbA1c as a biomarker for predicting C-sections ( 39 ). Meanwhile, our results also indicated that normal-weight women with HbA1c levels≥5.5% (37mmol/mol) and underweight women with HbA1c 5.1%-5.4% (32mmol/mol – 36mmol/mol) had an increased risk of primary C-section.…”

“…This is consistent with Chatzakis et al [10], who found an increased risk of high birth weight in patients with abnormal fasting glucose only. Relevant studies are consistent with this conclusion, Sesmilo G et al [4] con rmed FPG is an early marker of GDM, Zhou Zet al [11] demonstrated higher FPG was more strongly linked to adverse pregnancy outcomes among GDM patients, both linearity and Non-linearity of associations between glucose and complications should be taken into account.…”

“…Although there are no uniform conclusions about the specific mechanisms underlying this association, there may be a bidirectional link between anxiety or depression and hyperglycemia (Pan et al, 2010). A higher OGTT value is strongly associated with a higher degree of insulin resistance (Ghio et al, 2012) and a higher risk of adverse pregnancy complications (Zhou et al, 2020), both related to mental health disorders (Rustad et al, 2011). More clinical research is needed to confirm this relationship's potential mechanisms further.…”
