Sjogren's Systemic Clinical Activity Index (SCAI) a systemic disease activity measure for use in clinical trials in primary Sjogren's syndrome

Abstract: This initial evaluation supports the potential for the SCAI as a tool for systemic activity assessment in patients with PSS but additional work is required to assess sensitivity to change in clinical therapeutic trials.

“…The first 2, the Sjö gren's Syndrome Disease Activity Index (SSDAI) from Italy (1) and the Sjö gren's Systemic Clinical Activity Index (SCAI) from the UK (2), were both developed from exploratory studies conducted in a single country. To develop a consensus disease activity index, the European League Against Rheumatism (EULAR) has promoted a large international collaboration between primary SS experts, including those involved in the development of previous primary SS activity indexes (1,2). The resulting EULAR Sjö gren's Syndrome Disease Activity Index (ESSDAI) is a new clinical index modeled on physician's judgment of disease activity (online at http://www.eularsjogrenactivityindex.com) and was designed to measure disease activity in patients with systemic complications of primary SS (3).…”

“…The association between MPV and rheumatic diseases, such as systemic sclerosis (2), familial Mediterranean fever (3), rheumatoid arthritis (4), ankylosing spondylitis (5) and Behçet's disease (6) has been previously reported. In Sjögren's syndrome (SS), acute phase reactants are not included in the activity indexes, such as Sjögren's syndrome disease activity index (SSDAI) and Sjögren's systemic clinical activity index (7,8). Therefore, reliable parameters correlating with disease activity are needed.…”

“…The SCAI is an ordinal transition scale, derived from the BILAG scoring system, developed in a cohort of 104 patients [25,27]. It reflects changes in clinical symptoms during the 4-week period prior to evaluation, or compared to the previous visit.…”

“…Different disease-specific indexes have first been developed for evaluation of symptoms (subjective features) in the patients, such as Profile of Fatigue and Discomfort (PROFAD) and Sicca Symptoms Inventory (SSI) [22,23], and, more recently, for systemic features, such as SS disease activity index (SSDAI) [24] and Sjögren's Systemic Clinical Activity Index (SCAI) [25]. At the same time, disease damage indexes had been proposed to measure the chronic consequences of consecutives disease flares [24,26].…”
