Sleep problems and worrying precede psychotic symptoms during an online intervention for psychosis

Abstract: Objective. Experience sampling assessments (multiple assessments per day for approximately one week) indicate that positive symptoms fluctuate over time in psychosis. Precursors, such as sleep problems or worrying, predict these fluctuations. To date, it remains unclear whether the same precursors predict symptom variability also during treatment in an online intervention for psychosis, using assessments lying temporally further apart. Methods. Participants completed brief intermediate online self-report asses… Show more

“…These transient subclinical states of emotional distress could be the vehicle through which catastrophizing impacts on pain mental health (Sullivan et al, 2001 ). This is in line with a breadth of recent research on worry and mental health (Freeman et al, 2011 ; Sun et al, 2019 ; Bendau et al, 2020 ; Lüdtke et al, 2021 ). Similarly, the similar levels of general distress compared to March accompanied by the reduced perceived risk of contagion suggest that financial and additional worries (measured by the negative mindset scale) contribute to general distress after the initial uncertain phase of the pandemic has passed.…”

“…To ensure a high completion rate of the survey we limited the number of protective and risk factors. We did not ask about sleep quality, though sleep quality is beneficial for mental health, e.g., Lüdtke et al ( 2021 ). We used an abbreviated version of the CAPE positive scale and the short form of the clinical outcome (CORE) measure.…”

“…Furthermore, since anxiety symptoms are not significantly associated with SI in regression analysis, according to this study, anxiety symptoms might not need to be controlled when considering the association between sleep and SI. Despite our findings here, we submit that sleep has been found to be closely related to depression, anxiety and other mental health problems [ 24 , 25 ], and the vicious cycle of interaction between sleep and mental health problems might lead to increased suicidality, which we believe warrants further attention and investigation.…”

“…This was accepted by most patients and their relatives. There is increasing evidence supporting the effectiveness of online interventions both in patients with FEP (Arnold et al, 2019;Lüdtke et al, 2021;Ludwig et al, 2021;Mcenery et al, 2019) and their relatives (Lobban, Akers, et al, 2020;Sin et al, 2014;Stjernswärd & Hansson, 2016), what suggests that it is a therapeutic strategy that is effective and well accepted by patients.…”
