Small cell nonkeratinizing carcinoma of the cervix associated with acth production

Abstract: The case of a 40-year-old woman with primary small cell carcinoma of the cervix is reported. She developed widespread metastates and florid Cushing's syndrome. Serum ACTH levels were greatly elevited and no site of production other than the tumor could be demonstrated at autopsy. The tumor cells demonstrated features characteristic of cells of the APUD series. Such cells have been demonstrated in normal cervical epithelium; it is likely that they may become malignant, giving rise to tumors with a potehtial to … Show more

“…Many authors have reported cervical carcinomas containing endocrine cells (Tateishi et al 1975, Albores-Saavedra et al 1979, Miles et al 1982. Some of these tumours elaborated hormonal products such as serotonin, ACTH, p-MSH and insulin (Kiang, Bauer & Kennedy 1973, Jones et al 1976, Matsuyama et al 1979, Lojek et al 1980. Most of these neoplastic processes are not true carcinoids but carcinomas exhibiting focal endocrine differentiation.…”

“…De hecho, constituye el tipo tumoral menos prevalente dentro de los carci- 2 refirieron el primer caso de localización larín-gea, incidiendo en su identidad histológica con el carcinoma de células pequeñas de localización pulmonar. Asimismo, se han descrito otras localizaciones extrapulmonares, a saber: piel, páncreas y cérvix uterino, [3][4][5] con incidencias (en torno al 4%) muy por debajo de las descritas para el pulmón.…”

“…Subsequently, other cases were reported under various terms, such as "apudoma" [8], "argyrophil carcinoma" [8,9], "oat cell carcinoma" [10], "neuroendocrine carcinoma" [2], and "small cell non-keratinizing carcinoma" [11]. These neoplasms demonstrated a broad spectrum of differentiation ranging from well-differentiated forms resembling carcinoid tumors to poorly differentiated forms exhibiting a histologic image comparable to small-cell carcinoma of the lung [12][13][14].…”

“…Anti-neuron specific enolase (NSE) Owing to the rarity of hormone-secreting neoplasms in the cervix, only a few cases of cervical neuroendocrine tumors have been described. Some of these tumors elaborated hormonal substances such as ACTH, Beta-melanocyte-stimulating hormone (Beta-MSH), insulin, serotonin, and calcitonin [11,15,16]. Pearse [171, who introduced the APUD concept (amine precursor uptake and decarboxylation), believed that this category of cells originated from the neural crest and that they shared a number of ultrastructural and cytochemical characteristics 118].…”
