Aim Facial aging is the union of multiple effects such as exposure to the sun, ambiental stress, smog, tobacco. It reflects the dynamic, cumulative effects of time on the skin, soft tissues, and deep structural components of the face showing skin textural changes and loss of facial volume. There are a multitude of techniques currently used to perform face lifts, but the unique objective is to achieve a good and safe result. Methods This paper describes a new safe approach to midfacial rejuvenation that combines the elements of superficial musculoaponeurotic system (SMAS) plication, about four vectors, with a blepharoplasty to achieve longlasting improvement of the aging midface and a more natural effect. Plication of the SMAS over the suture, provides three vectors of elevation beneath the skin in midface rhytidectomy linked to the Lorè's fascia and the third vector of elevation of platysma behind the ear. Results This new technique, called KOr.U technique, was used in 31 patients between October 2010 and October 2012, producing effective long-lasting results, returning a volume of midface with no injury. Discussion This simplified approach is safe and can be easily performed under local anesthesia as an isolated midface procedure, avoiding the facial nerve.

Introdução: A cirurgia plástica facial é uma das áreas mais difíceis de se prover experiência prática para os residentes de otorrinolaringologia. Complicações de ritidoplastia têm sido amplamente relatadas na literatura. Objetivo: Este estudo avalia a incidência de complicações de ritidoplastias realizadas por residentes, sob supervisão adequada de um cirurgião experiente, em um Serviço de Residência Médica em Otorrinolaringologia. Forma de estudo: Análise de prontuários. Material e método: Os prontuários de 62 ritidoplastias consecutivas realizadas entre março de 1999 e março de 2001 foram retrospectivamente revisados. Foram obtidas informações sobre o paciente, o procedimento cirúrgico e as complicações pós-operatórias. Resultado: Os índices apresentados foram de 12,9% dos pacientes com complicações isoladas em resultado da cirurgia e 6,5% com mais de uma complicação. Incluem 1 hematoma volumoso (1,6%), 5 hematomas leves (8,1%), 3 seromas (4,8%), 2 deiscências de sutura (3,2%), 1 infecção (1,6%), 1 fístula salivar de parótida (1,6%), 1 deformidade de lóbulo de orelha (1,6%) e 1 parestesia de pavilhão auricular (1,6%). Não ocorreu nenhum caso de paralisia facial, eventos cardiopulmonares ou óbito. Houve apenas uma paciente do sexo feminino que expressou insatisfação com a cirurgia (1,6%). Conclusão: Apesar destes dados serem clinicamente relevantes, o tamanho da amostra é pequeno para se chegar a conclusões estatisticamente significantes. Apenas podemos sugerir que o ensino da ritidoplastia no ambiente acadêmico é eficiente e seguro, desde que realizado cuidadosamente pelo residente, com domínio teórico da técnica e da anatomia e sob supervisão de um cirurgião experiente.
