Since 1988, treatment strategies for our sarcoma patients have been determined by the same team and operations performed by one surgeon. The aim of this study was to analyse prognostic data on local recurrence and survival of 101 consecutive patients who presented in our institution with the primary tumour manifestation. After a median follow-up of 35 months, the local recurrence rate was 13.5%, the mean survival time was 68 months and the 5-year survival rate was 83%. Besides positive lymph nodes (only 3 patients) the quality of resection significantly influenced local recurrences (P < 0.05). Univariate predictors of mortality were tumour grade (P < 0.01), tumour size (P < 0.05), distant metastases (P < 0.01), and resection quality (P < 0.01). Multivariate predictors of mortality consisted of grade (P < 0.0001), positive lymph nodes (P < 0.001) and resection quality (P < 0.01). In this homogeneous group of patients, excellent recurrence and survival rates could be achieved. An optimized surgical treatment not only reduces the rate of local recurrences but also augments survival time.

IntroductionSoft tissue sarcomas are rare tumours that are infrequently seen in most surgical practices. They can occur in any anatomic region of the body. The size of a soft tissue sarcoma is an important prognostic variable and so affects the quality of tumour resection.Case presentationA 25-year-old Nigerian African presented with recurrent huge fibrosarcoma measuring about 55 × 40 × 10 cm at his posterior trunk. The patient's clinical condition was poor; the tumour seemed unresectable and the patient looked inoperable. He had an extensive excision of the tumour but could not afford adjuvant therapies. He was discharged home against medical advice but may succumb to metastases.ConclusionSarcomas in black people can present as extremely large masses; the dilemma in management is not only limited to the delay in presentation but also the poor socio-economic status of the patients and the frequent non-availability of supporting services. Treatment grants or subsidies from government may go a long way to ensuring that patients receive appropriate care.

SUMMARYObjective: The main objective was to identify the clinico-pathologic-picture and the influence on prognosis of some variables. Material and methods: We analized retrospectively the files of 44 adults patients with soft -tissue sarcomas managed at Belen Hospital, Peru, between 1967 and 1994. Estimation of survival was made by the method of Kaplan -Meier and to assesss the prognostic variables we used the log-rank test. Results: The median age was 46± 19 years (range, 16 to 85 years) and there were 21 males and 23 females (M:F, 0.9:1). The most common complaints were tumor and pain. Twenty -three tumors (52%) were located in the lower and 9 (21%) in the upper extremities. Most patients were classified in UICC stage II (48%), followed by stage III (23%), stage IV (18%) and stage I (11%). The majority of tumors were fibrosarcomas (34%), liposarcomas and rhabdomyosarcomas (14%). Thirteen tumors (30%) were classified as high -grade, 24 (55%) as intermediate -grade and 7 (16%) as low -grade. Twenty -seven patients underwent surgery, 13 received adjuvant chemotherapy and/or radiotherapy, and 4 were treated with primary chemotherapy and/or radiotherapy with 5 -year survival rates of 54%, 11 %, and 0%, respectively. The 5 -year survival rate for the entire group was 38%. According to an univariate analysis of the dependence of survival on variables, the following factors were significant: Age (p < 0.01), lenght of disease (p < 0.001), site (p < 0.001), lymph node status (p< 0.001), histologic grade (p< 0.01), clinical stage (p < 0.001) and type of treatment (p <0.001). Conclusion: The major factor in survival outcome is the ability to completely resect lesion.
