Solid-pseudopapillary tumour of the pancreas as a rare cause of gastric outlet obstruction: a case report

Abstract: The presence of a large bulky pancreatic tumour in a young female should raise suspicions of the diagnosis of solid-pseduopapillary tumour of the pancreas. This rare tumour has the characteristics of a low-grade malignancy with indolent behaviour. Most patients present with vague non-specific abdominal pain resulting in delayed diagnosis. The light microscopic features show solid areas alternating with pseudopapillary formations. Metastases are frequently amenable to resection.Favourable prognosis with long-ter… Show more

“…In our patient the tumor caused more or less acute occurring and in fact persisting abdominal pain, which is observed in most cases of Frantz tumor [2,11,19,24,27,29]. In some cases the entity is discovered, e.g., after minor injuries [20], or in connection with palpable abdominal masses [2,7,14,18,20,23,29], loss of weight, further on nausea, vomiting [13,18,24] to hematemesis [2,13] in extremely rare cases. There are also cases in which patients do not show symptoms at all, so that the tumor is then found incidentally [6,14].…”

“…Our decision to opt for a radical surgical therapy rather than FNA was made in agreement with Lévy et al [16]. For surgical management, due to tumor position (Caput pancreatis) and according to the experience, that by resection in toto (R0 -Resection) recovery is seen in about 95% of cases [7,10,24], Kausch-Whipple Operation (modified by Traverso-Longmire) was chosen, which is mentioned by many authors to be the method of choice in tumors of the pancreatic head [2,6,12,14,18,22,30]. Advantages of this procedure were radical and secure removal of the huge tumor on one hand, and to preserve the patient's stomach and its function on the other hand, which, according to clinical course paid off.…”

“…SPT of the pancreas is a relatively rare disease with differing opinion on management in literature 1 2. The origin of tumour has not been clarified and is argued that it originates either from ductal epithelium, acinar cells or endocrine cells.…”
