Solitary bone metastasis to the tibia from colorectal cancer- A case report

Abstract: The onset of osseous metastases during the course of colorectal cancer is not common. Although rare, they usually appear in the axial skeleton. In our report, we refer to the case of a 48-year-old patient who presented with colon cancer and eventually developed a solitary bone metastasis in the upper end of left tibia. At the time of diagnosis and staging investigations, the patient had only a primary disease.

“…An additional, frequently deployed treatment modality is chemotherapy. The administration of zoledronic acid has been reported in only 1 case [ 11 ]. Surgical excision of skeletal metastases has shown variable results [ 19 ], but the femoral amputation for the tibia metastasis in our case has been potentially curative.…”

“…The incidence of bone metastasis is reported in the English literature to be up to 4.7% in clinical cases 5,6 and up to 10.7% in autopsy cases. However the characteristics of the primary cancers, as well as the 6 pattern of bone metastasis remain unclear.…”

“…The patient underwent low-anterior resection and liver tumor resection. Pathological examination confirmed pT4bN2bM1a, stage IVa cancer, and 12 cycles of Avastin-FOLFIRI adjuvant chemotherapy (bevacizumab, leucovorin-calcium, fluorouracil, irinotecan hydrochloride) were administered, followed by the leucovorin, fluorouracil and oxaliplatin (FOLFOX) 7 regimen for tumor recurrence 1 month later. The patient tolerated chemotherapy well and was asymptomatic until July 2018, when she came to our clinic with a chief complaint of left leg pain that was aggravated by standing or walking.…”

“…In autopsy case series, incidence rates of 10.7% to 23.7% were observed. 1,2,[4][5][6][7] Considering the anatomical proximity of the colon to the paravertebral venous plexuses, the most common bone metastatic regions are the lumbar and sacral vertebrae, and the pelvis. 4 There are few reported cases of isolated solitary tibial metastasis, 2,4,7 and simultaneous bilateral solitary tibial metastases in colorectal cancer are even rarer.…”
