Solitary Intracranial Extra-osseous Plasmacytoma Presenting with Ophthalmic Signs

Abstract: Solitary plasmacytomas rarely develop in the skull, meninges, or brain. Ophthalmic signs as the initial manifestations of solitary intracranial plasmacytoma have rarely been described. We report the neuro-ophthalmologic, imaging, and pathologic findings for two patients. One patient presented with optic neuropathy, the second with bilateral sixth nerve palsies. Plasmacytoma is a treatable intracranial tumor that should be considered in the differential diagnosis of patients who present with optic neuropathy or… Show more

“…Plasmacytoma can be successfully managed with involved field irradiation with 50 Gy, preceded by chemotherapy in patients with large lesions. 74 WBRT 35-45 Gy followed by a 5-15 Gy boost was proposed for CNS Hodgkin lymphoma, with a median OS from CNS involvement of 44 months. 75 …”

“…Isolated intracranial plasmacytoma has been described as responsible for compressive optic neuropathy at the orbital apex in a small number of single case reports. [3][4][5][6] Orbital plasmacytoma, also a rare condition, may potentially involve the optic nerve in this location. [7][8][9] Infiltration by myelomatous cells is an even more unusual mechanism of optic nerve damage.…”
