Sonographic Diagnosis of a Subclinical Wandering Spleen

Abstract: A wandering spleen is a rare condition. It is usually diagnosed when abdominal pain develops secondary to splenic torsion. Although splenic hypermobility is the pathognomonic feature of a wandering spleen, it is rarely revealed by imaging in the subclinical stage. We report 3 patients with a subclinical wandering spleen who had incidental sonographic findings of splenomegaly. Gray scale and color Doppler sonography in the right decubitus position can easily show the migratory nature and perfusion status of a w… Show more

“…Abdominal ultrasound are using to measure the spleen's dimension, to evaluate the echogenicity of the splenic parenchyma. In Doppler ultrasound, the decrease or missing of blood flow in splenic artery or spenic vein are suggestive of torsion of vascular pedicle [7,8]. In our case, the diagnosis of splenic torsion by abdominal ultrasound was difficult due to abdominal air.…”

“…The diagnosis can be confirmed by abdominal ultrasound, abdominal pelvic CT scan or abdominal magnetic resonance imaging (MRI). In addition, Chen has been reported the advantage of an abdominal ultrasound for the diagnosis of a subclinical wandering spleen [7]. Abdominal ultrasound could confirm the diagnosis by showing the vacuity of the splenic lodge.…”

“…In addition to a clinical and ultrasonographic evaluation, we have used also a MR protocol. The literature suggests that Doppler ultrasound is used to check the spleen position and parenchymal flow [ 20 23 ]. Some authors describe CT scan as the best choice to identify this rare condition [ 24 ], although Buckley et al [ 25 ] suggest that MR imaging may confer certain advantages because of its lack of ionizing radiation, as well as its superior tissue characterization, and it is not operator dependent.…”

“…Ligamentous laxity can be acquired due to conditions such as splenomegaly or pregnancy; however, it is often congenital 3 . Many cats with a wandering spleen have symptoms such as abdominal pain, but asymptomatic cases have also been reported 4 . Treatment of the wandering spleen in humans involves the risk of torsion; hence, surgeries such as splenectomy or gastropexy are commonly performed 5 …”
