Background: Amniotic fluid index (AFI) is a key element in fetal biophysical profile that predicts pregnancy outcome especially near term in primigravidae. The objective of the study was to determine a reference range of AFI in primigravidae of a local population having gestational age from 37 to 42 weeks. Methodology: This study was carried out from January 2019 to December 2019 after approval from Institutional Review Board and Ethical Committee at Shifa International hospital, Islamabad, Pakistan. Applying inclusion and exclusion criteria, a sample of 272 was selected. AFI was calculated for each subject using ultrasound and recorded on a data collection sheet. For the sake of simplicity and clinical relevance, the population was further stratified as term pregnancy (gestational age 37-39 weeks) and postdates pregnancy (40-42 weeks). The reference range of AFI was then calculated using mean ±two standard deviations. Results: The reference range of AFI for gestational age of 37-42 weeks (population as a whole) was 6.8- 17.2 cm. For term pregnancy, it was 7.5- 17.5 cm, and for postdate pregnancy, it was 5.8 to 15.4 cm. Conclusion: The reference range of AFI for gestational age of 37 to 42 weeks in Primigravida women was 6.8 to 17.2 cm. There is a decreasing trend in AFI as pregnancy advances from term to postdate. Objective: The outcomes and management of low amniotic fluid index (AFI) in pregnancy are controversial. The purpose of this study was to determine the relationship between low AFI and perinatal outcomes. Materials and methods: This prospective study was conducted on 420 uncomplicated singleton pregnant women with a gestational age of over 28 weeks who referred to Al-Zahra Hospital in Rasht (Iran) for routine perinatal care. Pregnant women were divided into 3 groups of 140 patients based on the AFI and were followed up until delivery. Three groups included normal (8<AFI≤25 cm(, borderline )5<AFI≤8 cm( and oligohydramnios (AFI≤5cm( AFI. Results: The three adverse outcomes of respiratory distress, hospitalization in NICU, and length of hospitalization were statistically significantly different between the two groups with normal and borderline AFI and in the borderline group was more than the normal group. Adverse outcomes including; low birth weight (LBW), small for gestational age (SGA), respiratory distress, 1- min APGAR scores<7, hospitalization in NICU and its duration were statistically significantly different between the two groups with normal AFI and oligohydramnios, and it was more in the oligohydramnios group than the normal group. The three adverse outcomes of LBW, SGA and1- min APGAR scores<7 in the two borderline and oligohydramnios groups had statistically significant differences and were more in the oligohydramnios group than the borderline group. Conclusion: Consideration to the AFI in perinatal care to predict adverse perinatal outcomes and perform necessary interventions to improve these outcomes is necessary.

Objective: To compare the frequency of diagnosis, induction of labour and caesarean section in patients with isolated oligohydramnios diagnosed by amniotic fluid index versus single deepest vertical pocket. Study design: Randomized controlled trial Place and duration of study: Department of Obstetrics and Gynecology, Holy Family hospital, Rawalpindi from March 2020 to August 2020. Materials & Methods: A total of 110 (55 in each group), 18 to 35 years of age of parity <5 were included. Group A females were evaluated by using AFI and group B females were evaluated by using SDVP. Patients were managed according to standard protocol practiced in the department. Caesarean section was performed in case of fetal distress, in presence of meconium stained liquor or in case of failure to progress of labour. Results: In my study, oligohydramnios was recorded in 19/55 (34.50%) in group A (amniotic fluid index) versus 11/55 (20.0%) in group B (single deepest vertical pocket) (p-value = 0.086). Similarly induction of labour was recorded in 19/55 (34.50%) in group A (amniotic fluid index) versus 11/55 (20.0%) in group B (single deepest vertical pocket) (p-value = 0.086) and caesarean section was recorded in 16/55 (29.09%) in group A (amniotic fluid index) versus 07/55 (12.73%) in group B (single deepest vertical pocket) (p-value = 0.035). Conclusion: This study concluded that frequency of diagnosis, induction of labour and caesarean section in patients with isolated oligohydramnios diagnosed by amniotic fluid index is higher as compared to single deepest vertical pocket.
