Background: Preoperative neck ultrasonography (US) may detect nodal metastases of papillary thyroid carcinoma (PTC) but its utility in detecting metastases at specific neck subsites and levels is not known. We therefore evaluated preoperative US in detecting cervical metastases of PTC according to neck subsites and levels. Methods: Preoperative US was performed in 133 new patients to detect metastases at three central cervical subsites and five lateral cervical levels. All patients underwent total thyroidectomy and bilateral central neck dissection. Thirty-four patients with lateral nodal metastases underwent modified radical neck dissection. Results: Lymph node metastases to the central and lateral cervical compartments were identified in 57.9% and 25.6%, respectively. The sensitivity and specificity of US for detecting central nodal metastasis were 61.0% and 92.8%, respectively. In the lateral neck, US detected nonpalpable lymph node metastases in 6 of 34 patients (17.6%). Overall, US was >85.0% specific at all cervical subsites and levels. Conclusion: Preoperative US may detect cervical metastases of PTC and may assist in determining the necessity and extent of neck dissection in PTC patients.

Background: The objectives of this study were to evaluate thyroid lesions, to differentiate between possible benign and malignant lesions and to assess the diagnostic accuracy of high resolution sonography in thyroid lesions. Methods: Patients presenting with clinical suspicion of thyroid disease referred to the Department of Radiodiagnosis were included in the study. Ultrasound was done as an initial diagnostic imaging method and later underwent to ultrasound guided FNAC, for confirmation of diagnosis. Findings of the thyroid ultrasound scan were correlated with FNAC diagnosis. Results: Ultrasound was able to pick up lesions in all 100 cases with a sensitivity and specificity of 72 %and 100% respectively. Ultrasonography showed a Diagnostic accuracy of 97% for thyroid diseases. Sonography could considerably differentiate between malignant and benign lesions. Ultrasound guided FNAC showed high diagnostic yield for detection of thyroid diseases. Conclusion: The current study suggests that high resolution sonography can be used as a primary imaging modality for the evaluation of thyroid diseases with high diagnostic accuracy. Ultrasound helps in guiding FNAC for confirmation of the diagnosis.

Whereas in the past a negative diagnostic 131 I whole body scan (WBS) was interpreted as the lack of significant residual or recurrent thyroid cancer, today the patient with negative WBS and measurable serum thyroglobulin (Tg) presents a diagnostic and therapeutic dilemma. Previous studies have shown a high rate of visualization of uptake and a decrease in Tg after one or more therapeutic doses of 131 I. In order to further assess the significance of this finding, retrospective analysis of patients with persistent thyroglobulinemia and negative WBS was performed for evidence of surgically amenable disease. Seven out of seventeen patients had neck ultrasound and/or computerized tomography (neck Ϯ chest) showing the presence of pathologically confirmed malignant masses ranging from 1 to 4 cm in size. Their serum Tg while on L-thyroxine ranged between 2.4 and 1173 pmol/l. Removal of the identified masses resulted in a greater than 75% reduction in serum Tg in four out of five patients in the group. One patient achieved a serum Tg of < 1.5 pmol/l while hypothyroid. Empiric 131 I treatment of eleven patients with persistent thyroglobulinemia resulted in demonstrated uptake on post-therapy scan in seven. Further study is needed to compare the efficacy, safety and cost of a diagnostic approach to radiologically identify and surgically resect identified disease versus empiric therapeutic 131 I treatment and high-dose WBS in this group of patients. Patients with negative WBS and persistent thyroglobulinemia, even to levels < 4.5 pmol/l, may have significant foci of thyroid cancer in surgically accessible areas. This suggests the need for a redefinition or clarification of the term 'recurrence' in thyroid cancer.
