BackgroundThe negative consequences of prescription opioid misuse and opioid use disorder make it relevant to identify factors associated with this problem in individuals with chronic pain. This cross‐sectional study aimed at identifying subgroups of people with chronic pain based on their psychological profiles, prescription opioid misuse, craving, and withdrawal.MethodsThe sample comprised 185 individuals with chronic pain. We performed hierarchical cluster analysis on impulsivity, anxiety sensitivity, pain acceptance, pain intensity, opioid misuse, craving, and withdrawal.ResultsThe four‐cluster solution was the optimal one. Misuse, craving, and anxiety sensitivity were higher among people in the Severe‐problems cluster than among people in the other three clusters. Withdrawal was the highest in the High‐withdrawal cluster. Impulsivity was higher among people in the Severe‐problems and High‐withdrawal clusters than those in the Moderate‐problems and Mild‐problems clusters. Pain acceptance was higher among people in the Mild‐problems cluster than among people in the other three clusters. Anxiety sensitivity and misuse were higher among people in the Moderate‐problems cluster than among people in the Mild‐problems cluster.ConclusionsThese results support that impulsivity, anxiety sensitivity, and pain acceptance are useful constructs to identify subgroups of people with chronic pain according to their level of prescription opioid misuse, craving, and withdrawal. The results of this study may help in selecting the early intervention most suitable for each of the identified profiles.SignificanceThe psychological profile of individuals with chronic pain, prescription opioid misuse, craving, and withdrawal is characterized by fearing anxiety‐related symptoms due to the catastrophic interpretation of such symptoms and reacting impulsively to negative moods. In contrast, participants with high pain acceptance had less prescription opioid misuse, craving, and withdrawal. The profiles identified in this study could help clinicians select targets for intervention among profiles with similar needs and facilitate early interventions to prevent opioid misuse onset or aggravation.

Tests are fundamental to psychology, and their use should always be supported by solid evidence. Since 2010, the National Test Commission of the General Council of the Spanish Psychological Association has been carrying out annual evaluations, using the Test Evaluation Questionnaire-Revised (CET-R) with the collaboration of external experts. To date, 96 tests have been evaluated. This tenth edition includes six tests from three well-known publishing houses: TEA Hogrefe, Pearson Educación, and Giunti Psychometrics. In addition, the psychometric properties were also reviewed of a non-commercial test, the Geriatric Depression Scale (GDS), mentioned among the 25 most used tests by Spanish psychologists. Evaluating non-commercial tests, developed in academic contexts, is crucial, as it enriches the set of tools available to practitioners. This paper addresses the challenges of evaluating tests of this type and offers suggestions for improving both the development and validation of the tests as well as the review of their psychometric properties using the CET-R.

Diagnostic and Predictive Capacity of the Spanish Versions of the Opioid Risk Tool and the Screener and Opioid Assessment for Patients with Pain—Revised: A Preliminary Investigation in a Sample of People with Noncancer Chronic Pain

Introduction: Accurate assessment of the risk of opioid abuse and misuse in people with noncancer chronic pain is crucial for their prevention. This study aimed to provide preliminary evidence of the diagnostic and predictive capacity of the Spanish versions of the Opioid Risk Tool (ORT) and the Screener and Opioid Assessment for Patients with Pain-Revised (SOAPP-R). Methods: We used the Current Opioid Misuse Measure (COMM) as criterion measure to assess the capacity of each tool to identify patients misusing opioids at the time of the assessment. Eighteen months later, we used the COMM and the Drug Abuse Screening Test-10 (DAST-10) to assess their predictive capacity. In total, 147 people with noncancer chronic pain participated in the diagnostic study, and 42 in the predictive study. Results: Receiver operating curve analysis showed that the SOAPP-R had an excellent capacity to identify participants who were misusing opioids at the time of assessment (area under the curve [AUC] = 0.827). The diagnostic capacity of the ORT was close to acceptable (AUC = 0.649-0.669), whereas its predictive capacity was poor (AUC = 0.522-0.554). The predictive capacity of the SOAPP-R was close to acceptable regarding misuse (AUC = 0.672) and poor regarding abuse (AUC = 0.423). Conclusion:In the setting of Spanish-speaking communities, clinicians should be cautious when using these instruments to make decisions on opioid administration. Further research is needed on the diagnostic and predictive capacity of the Spanish versions of both instruments.
