Spectral Doppler of the Hepatic Veins in Rate, Rhythm, and Conduction Disorders

Abstract: Doppler interrogation of blood flow in the hepatic veins (HVs) provides valuable information regarding a wide spectrum of pathological processes that affect the right heart. Systematic analysis of the direction, velocity, and phasicity of the HV waveforms allows one to distinguish normal from abnormal patterns and provides important diagnostic information. Abnormalities in heart rate, rhythm, and intracardiac conduction are commonly encountered during echocardiographic studies. Sinus bradycardia and tachycardi… Show more

“…SFR can also be appreciated on CDI and on CMM. Whilst SFR is a very specific sign of severe TR, other conditions may result in SFR especially when there is atrial contraction against a closed TV as in the case of paced rhythms, atrioventricular dissociation and atrial arrythmias 18,19 . In this patient, the rhythm was paced and there was underlying AF.…”

“…This would suggest that could be reasonable to adopt different flexible approaches, rather than performing a comprehensive, time-consuming, protocol. Additionally, the hepatic veins in patients with rhythm disorders or with intracardiac devices might be difficult to interpret [33]. Therefore, selecting patients with a portal vein determination could be enough, as supported by other studies [18][19][20]24,34].…”
