Speech and language impairments in behavioral variant frontotemporal dementia: A systematic review

Abstract: Although behavioral variant frontotemporal dementia (bvFTD) is classically defined by behavioral and socio-emotional changes, impairments often extend to other cognitive functions. These include early speech and language deficits related to the disease’s core neural disruptions. Yet, their scope and clinical relevance remains poorly understood. This systematic review characterizes such disturbances in bvFTD, considering clinically, neuroanatomically, genetically, and neuropathologically defined subgroups. We i… Show more

“…Similarly, language disturbances commonly co-occur with the impairment of the socio-emotional behavior in bvFTD. Severity of language deficits seems to be linked with disease severity and frontotemporal atrophy suggesting a close link between bvFTD, speech and language deficits, and disease's core neural disruptions [53]. Finally, we identified a third component, mainly loading to visuo-spatial executive tests.…”

“…Evidence from other world regions reveals general patterns of affected and spared linguistic functions across bvFTD cohorts, with marked variability for some domains (23). Available results come mainly from studies from North America, Western Europe and Australia, with a marked predominance of English over other languages.…”

“…Of note, a number of patients do not fit criteria for any of the three core syndromes, often called PPA-unclassified, atypical PPA, or PPA-not otherwise specified (PPA-NOS) [10][11][12]. Features of each of the language variants have been described in people with bvFTD, but few studies have been performed to investigate the type or extent of linguistic impairment in bvFTD, and little is known about whether there are particular clinico-pathological associations for any of the symptoms [8,13,14].…”
