Spider bite‐induced pagetoid reticulosis?

Abstract: In summary, solitary PR should always be considered in the differential diagnosis of striking epidermotropic forms of cutaneous T-cell lymphoma. We believe that if careful attention is given to the clinical, histologic and phenotypic finding the differentiation between PR and MF can be made. It seems that the clinical behaviour is less predictable than initially thought and that aggressive local management and close long-term follow-up are warranted.

“…Often atypical T‐cells are found along the dermoepidermal junction, in the dermis, and within Pautrier's microabscesses in MFPP and classical MF, while our analysis revealed minimal dermal infiltrate in WKD 25 . Importantly, the phenotypic expression of atypical T‐cells in MF is predominantly CD4+/CD8‐ while CD4‐/CD8+ T lymphocytes are more common in WKD 13 . With the exception of its disseminated subtype, WKD is a localized cutaneous pathology with no risk of extracutaneous transformation into lymphoma, which is more likely in classic MF 16,22 …”

“…Pruritus, for example, was described in a case of WKD on the plantar aspect of the foot 12 . The pain was a presenting symptom in a patient who reported a spider bite on the hand that preceded the cutaneous eruption of the lesion 13 . Less commonly, it may exhibit locally aggressive behavior without dissemination, even doubling in size over the course of 3 months 14 .…”
