“…It has been demonstrated that the etiology of arachnoiditis includes an initial, noxious insult to the pia-arachnoid mater, which induces inflammation, followed by adhesion formation. 5,6 Once symptomatic arachnoiditis occurs, its treatment is refractory; and therefore, during the intradural maneuver, the gentle retraction of nerves, complete hemostasis, removal of subarachnoid clot, and successful closure of dura mater may be important to prevent arachnoiditis. Meanwhile, this novel approach can preserve arachnoid membrane without opening of the subarachnoid space, resulting in alleviation of the direct insult to the subarachnoid components and a reduced risk of arachnoiditis.…”
