Spinal Hydatid Cyst Mimicking Arachnoid Cyst on Magnetic Resonance Imaging

Abstract: Background: Primary spinal hydatid cysts are uncommon and account for 1% of all cases of hydatid disease. Echinococcus granulosus is most often responsible for the cyst hydatid. Intradural, extramedullary involvement is rare. When the cysts do not demonstrate typical magnetic resonance imaging findings, the differential diagnosis is more complex. Method: Case report. Findings: An isolated primary hydatid cyst of the spine in a 35-year-old man that appeared to be an arachnoid cyst on preoperative radiographic e… Show more

“…The cyst wall thickness, presence of daughter vesicles, and germinal membrane's separation from the pericyste are the characteristic findings on MRI, which is one of the most useful diagnosis methods (6). In our case, daughter vesicles were typically seen in MRI.…”

“…6 An MRI is the imaging modality of choice. 38,47 An ultrasonography of the abdomen/pelvis and chest x-rays fairly rule out the disease at the most common sites. They are sausage like in shape with 2 dome-shaped ends and no debris in the lumen.…”

“…The presence of the markedly hypointense cyst wall on T1-weighted and T2-weighted MRI images is characteristic of hydatid disease. 38 To minimize the risk of recurrence, intraoperative irrigation with disinfectants like hypertonic saline, dilute betadine solution, or AgNO 3 after removal of the cyst has been recommended. 40 MR spectroscopy may be used as an adjunct to imaging both in the diagnosis of intraspinal hydatid cysts and in monitoring of drug therapy to differentiate a viable and fertile cyst from its inactive form.…”
