To the Editor:We read with great interest the article by Cuoco and colleagues, 1 entitled "Spinal Intradural Arachnoid Cysts in Adults An Institutional Experience and Literature Review." This retrospective analysis examined 29 patients with arachnoid cysts, including 27 with primary cysts and 2 with secondary cysts. Laminectomy combined with cyst fenestration was performed on 22 patients, encompassing both individuals with secondary arachnoid cysts. Postoperatively, thoracic back pain significantly subsided in all cases (P = .034). Surgical outcomes were notably successful in both primary and secondary arachnoid cyst groups.Although the reported outcomes are promising, the literature review within this article highlights a significant distinction between primary and secondary arachnoid cysts.
