The aim of the study: to determine the structure of dorsalgia and comorbid diseases and the possibilities of rehabilitation in elderly patients with degenerative-dystrophic diseases of the spine (DDSD). The results of observation of 120 elderly patients who received sanatorium treatment in the Urgench sanatorium during 2020-21, aged 60-74 years, were analyzed, including 71 women, 49 men. All patients underwent a neurological examination, X-ray, CT or MRI of the spine, to identify comorbid diseases - ECG, ultrasound of internal organs, consultations with a general practitioner, urologist or gynecologist. According to the literature data and the results obtained, when selecting elderly patients with dorsalgia for treatment, it is necessary to take into account the “red flags” and refer such patients to specialized specialists. In addition, other comorbid diseases are identified in the elderly, which may limit the treatment of drug and physiotherapy (rehabilitation) therapy, which must be considered. Finally, in the elderly, it is necessary to take into account the so-called “yellow flags” - cognitive and depressive manifestations that affect the subjective sensation of pain.

Back pain is a common reason for seeking primary care from older and younger people. Early spinal imaging by a therapist is indicated to eliminate potentially dangerous causes of pain. Independent magnetic resonance imaging or computed tomography examination at the patient’s initiative in the absence of “red flags” back pain in most cases does not come close to identifying the obvious cause of pain syndrome, and it creates the preconditions for the formation of an exaggerated picture of the disease in the patient himself, driving him into the “trap” of chronic pain. Attention of the polyclinic doctor to the clinical symptoms detailed in the article, following the standard algorithms of diagnosis will allow timely suspicion of “red flags” back pain, avoid hyperdiagnosis of the cause, and minimize the risk of its chronization. The algorithm for treatment of acute and chronic episodes of pain should include informing the patient about the favorable outcome in the vast majority of cases, optimizing the physical activity of the patient, the use of complex drug therapy, the basis of which are nonsteroidal anti-inflammatory drugs (predominantly selective), muscle relaxants and other pathogenetically based medicines.

Abstract. This article presents a comprehensive review of literature on four current non-invasive non-drug methods of treating herniated and protruded intervertebral discs: exercise therapy, high-intensity laser therapy (HILT), deep oscillation therapy (Hivamat), and high-intensity magnetic therapy (SIS therapy). The purpose of the literature review is to evaluate the efficacy, safety, and treatment outcomes of each therapy. A review of the available literature, including clinical trials, systematic reviews, and comparative studies, was carried out. The focus is on the effectiveness of each treatment method in reducing pain and improving mobility and overall well-being of patients. The results of the literature review have shown that all four treatment methods are promising for the treatment of herniated and protruded discs. Exercise therapy is effective in the long term in reducing pain and relapse rates. HILT demonstrates rapid pain relief and reduction of inflammation. Hivamat is distinguished by its effectiveness in pain relief and tissue restoration. SIS therapy offers a minimally invasive approach with targeted pain relief. The authors conclude that modern non-invasive treatment methods, such as exercise therapy, HILT, Hivamat, and SIS therapy, are an efficient alternative to traditional methods of treating herniated and protruded intervertebral discs. The results of the analysis indicate a potential shift in clinical practice towards the need to include non-pharmacological methods in comprehensive rehabilitation programs. However, further largescale studies are recommended to better understand their long-term effectiveness and optimal use in clinical settings.
