Background: A rare complication of diverticulosis of the colon is giant colonic diverticula (GCD). The condition was first described in English literature in 1953 [1, 2]. Methods: A Medline search was undertaken for English, French and German language articles on ’giant colonic diverticula’. Results: A total of 135 patients were identified, presenting with a total of 155 GCD [1–98]. With a complication rate of 28% and an operative mortality of 5%, GCD seems to have a high clinical significance. Radiological examination of choice seems to be plain abdominal X-ray and CT examination, barium enema carries the risk of perforation of the diverticulum, and should not be performed. Conclusions: There are different therapeutic options, in our opinion diverticulectomy alone is not the treatment of choice. Because of the possibility of recurrence and oncological reasons, colectomy seems to be the best treatment. The creation of a protecting colostomy depends on other operative findings.

Abstract. Spindle cell tumors and reactive proliferations of the thyroid gland are rarely reported. In this report, we described a case of follicular adenoma with spindle cell component. The spindle cell proliferation constituted more than 90% of the 4.0 cm lesion and showed fascicular pattern with focal areas of collagenous stroma and hyalinized blood vessels. There was a transition from follicle like glandular structure to fascicular pattern. There was no mitosis and necrosis. Immunoreactivity with thyroglobulin proved that the spindle cell proliferation is of follicular origin. Also strong thyroglobulin and cytokeratin positivity of glandular areas and the gradual decrease in positivity of spindle cells showed epithelial-mesenchymal transformation/spindle cell metaplasia of the thyroid. We thought that it is important to differentiate spindle cell metaplasia in follicular lesions, especially follicular adenoma, from malignant thyroid neoplasms.

Transformation of well-differentiated thyroid adenocarcinoma to anaplastic cancer has been occasionally reported. Two new cases of such transformation are now described. The time interval from diagnosis to the development of anaplastic cancer was 15 years in the first patient and 10 years in the second. Both patients ultimately developed extensive bilateral pulmonary metastases and died within 6 months from the diagnosis of the transformation. Hypotheses concerning the pathogenesis of the transformation and the literature were reviewed. We believe that histopathological confirmation of recurrent and rapidly progressive thyroid tumors is necessary prior to initiation of therapy.
