Spindle cell lipoma is strongly CD34 positive. An immunohistochemical study

Abstract: Spindle cell lipoma (SCL) is an uncommon soft tissue neoplasm that may provide diagnostic difficulty to the histopathologist. Four retrospectively identified SCLs were evaluated immunohistochemically with a broad panel of antibodies (CD34, factor XIIIa, S-100, actin and factor VIII). All four SCLs were strongly CD34 positive (95 to 100% of cells) and focally factor XIIIa positive (average 16% of cells). Tumor cells were S-100, actin and factor VIII negative. Dermatofibrosarcoma protuberans, neurofibroma, schwa… Show more

“…Although SCLs are benign tumors they are mostly mixed with malignant tumors like spindle cell liposarcoma, myxoid liposarcoma and well differenciated liposarcoma. In recent studies, all SCLs are detected as CD34 positive and actin, laminin and desmin negative (9). In present case the cells were immunopositive for CD34 and vimentin.…”

“…To our knowledge, this has not been reported previously. Previous immunohistochemical studies of spindle cell lipomas have mainly been guided to determine the expression of CD34, S-100 protein, and Bcl-2 [1,15,16,17]. We have not been able to find reports that include desmin.…”

“…They are composed of a mixture of bland spindle cells and mature adipocytes in variable proportions together with bundles of ropy collagen and scattered mast cells. Most immunohistochemical analyses have been limited to the study of S-100 protein, CD34, and Bcl-2 protein expression, with only some reports including a small number of other antigens [1,15,16,17]. This has hampered our knowledge of the complete immunophenotypical profile of spindle cell lipomas, which may induce to misdiagnosis when studying lesions with nonclassical histological features or those in unusual locations.…”

“…5 In addition, atypical lipomas are negative for CD34, unlike SCL. 6 SCL with prominent myxoid matrix may be difficult to differentiate from myxoid liposarcoma and myxofibrosarcoma. Myxoid liposarcoma is usually deep-seated, unlike SCL, and displays delicate branching vascular network along with lipoblasts.…”
