Splenectomy for Primary and Recurrent Immune Thrombocytopenic Purpura (ITP)

Abstract: Of 565 patients with thrombocytopenia admitted to Duke University Hospital between 1975 and 1985, 100 had splenectomy. Ninety-eight patients had failed chronic immunosuppressive therapy and three patients had acute intracranial bleeding or total absence of platelets in the peripheral blood smear, and had urgent splenectomy. At primary splenectomy, accessory spleens were identified and resected in 18% of patients. There was no operative mortality. Fifty-eight patients had an excellent response to splenectomy an… Show more

“…A splenectomy may lead to severe postoperative complications, including venous thromboembolism, serious infections, or bleeding [10], and even mortality, all of which are of even greater concern for elderly and frail patients [11,12]. Moreover, older ITP patients may be less likely to respond to splenectomy [8,13,14]. Our finding that rate of splenectomy was higher among patients with higher disease activity are consistent with the guidelines for cITP treatment.…”

“…In case of open splenectomy, Akwari et al 7 described nine patients undergoing open accessory splenectomy for recurrent ITP, showing complete remission in six patients. In laparoscopic approach, Szold et al 8 describe successful laparoscopic accessory splenectomy in eight patients with recurrent ITP.…”
