Spondyloarthropathy: diagnostic imaging criteria for the detection of sacroiliitis

Abstract: Diagnostic imaging is crucial to the diagnosis and monitoring of spondyloarthropathies. Magnetic resonance imaging is the most relevant tool for the early detection of sacroiliitis, allowing the institution of therapeutic strategies to impede the progression of the disease. This study illustrates the major criteria for a magnetic resonance imaging-based diagnosis of spondyloarthropathy. The cases selected here present images obtained from the medical records of patients diagnosed with sacroiliitis over a two-y… Show more

“… 27 30 MRI offers the additional advantage of being able to identify acute inflammatory changes related to bone marrow oedema and chronic inflammatory changes related to fatty atrophy. MRI is therefore more sensitive at detecting the early signs of axSpA, 27 , 31 , 32 and does not confer ionising radiation, thereby making it the imaging modality of choice in the 2009 ASAS guidelines. 27 Magnetic resonance enterography (MRE) of the small bowel is a type of oral contrast MRI scan used to aid the diagnosis and complications of IBD, principally small bowel CD.…”

“…The relevant findings in the evaluation of the Table 2-Sensitivity (95% CI), specificity (95% CI), positive predictive value, negative predictive value, and accuracy, by reader, for the T1 STIR Gd+ and T2 SPAIR sequences. sacroiliac joints include enthesitis, synovitis, capsulitis, and erosions (11) . The STIR sequence is just one of the many possible methods of fat suppression, which makes it possible to identify bone edema (12,13) .…”

“…The identification of sacroiliac joints inflammation is of great importance for the diagnosis of axial spondyloarthritis. The relevant findings in the evaluation of the sacroiliac joints include enthesitis, synovitis, capsulitis, and erosions ( 11 ) . The STIR sequence is just one of the many possible methods of fat suppression, which makes it possible to identify bone edema ( 12 , 13 ) .…”

“…The radiology literature of Brazil has recently focused on the role of MRI in the diagnosis of musculoskeletal diseases ( 22 - 30 ) . The assessment of osteoid osteoma with MRI could be helpful.…”
