Objective: To assess the factor contributed to abortion and to identify the physical and psychosocial effect of abortion on mothers. Methodology: A descriptive study was conducted at Baghdad teaching hospital and AL-Yarmok teaching hospital, purposive sample (100) mothers was selected. The instrument was designed and constructed by reviewing related literatures, and composed of three parts, part one for demographic and reproductive characteristics, part two for the causes contributed to abortion and part three for physical and psychosocial effect of abortion on women. Data were collected by personal interview in 20 March to 15 April \ 2012 and descriptive statistical procedures were used to analyze to data. Result: The result of the study showed the highest percentage (30%) of the study sample were ate age group between (30-34) years, (38%) of them were primary school graduated and (64%) of them were house wife, (34%) of them were multigravida, (28%) of them were primipara, (68%) of them have one abortion, (64%) of them have previous curettage and (65.6%) of them have curettage for one time. The revealed that the most causes of abortion were the physical tiredness and accidental trauma, most of the study sample suffering of physical effectiveness after abortion (74%) of them having chronic abdominal pain. The psychological result revealed majority of the study sample suffer of sadness, tiredness, emotional difficulties, eating disorders, depressive, while the social effect revealed increased family problem and violence behavior toward the husband and family. conclusion: According to the result we concluded that there were many factors contributed to abortion and psychosocial effects were more common than physical effects among mothers who had abortion. Recommendation: The study recommended routine screening for women to detect the causes, psychosocial support through educational programs and mass media especially for those women with recurrent abortion.

Objective: To determine the outcome of using tamoxifen with misoprostol in first trimester pregnancy termination. Materials & Methods: This study was a case series and conducted at Department of Gynaecology & Obstetrics from 1st April ,2018 to 30th September at Nishtar hospital Multan,2018 over the 6 months period .163 patients, 18 to 40 years of age with fetal demise or missed abortion confirmed on ultrasonography in 1st trimester (<12 gestational weeks) of pregnancy were included in the study. All the enrolled patients received 20 mg tamoxifen orally for 4 days then it is followed by 800 mcg oral misoprostol for 1 or 2 days orally (maximum 80 mg) and the outcome (in terms of complete abortion) was assessed after 7 days. Results: Mean age in our study was 24.89 ± 5.23 years. Mean gestational age was 7.07 ± 3.67 weeks. Majority of patient i.e. 49.69% were nulliparous. Complete abortion after tamoxifen with misoprostol therapy was seen in 149 (91.41%) cases. Conclusion: This study concluded that tamoxifen with misoprostol in first trimester pregnancy termination is very effective and useful with 91.41% rate of complete abortion.

Preconception Mental Health Predicts Pregnancy Complications and Adverse Birth Outcomes: A National Population-Based Study

Pregnancy complications and poor birth outcomes can affect the survival and long-term health of children. The preconception period represents an opportunity to intervene and improve outcomes; however little is known about women’s mental health prior to pregnancy as a predictor of such outcomes. We sought to determine if and to what extent women’s preconception mental health status impacted subsequent pregnancy complications, non-live birth, and birth weight using a nationally representative, population-based sample. We used pooled 1996-2006 data from the nationally-representative Medical Expenditure Panel Survey (MEPS). Poor preconception mental health was defined as women’s global mental health rating of “fair” or “poor” before conception. Logistic regression was used to assess the association between preconception mental health and pregnancy complications, non-live birth, and having a low birth weight baby within the follow up period. Poor preconception mental health was associated with increased odds of experiencing any pregnancy complication (AOR 1.40, 95% CI: 1.02-1.92), having a non-live birth (AOR 1.48, 95% CI: 0.96-2.27), and having a low birth weight baby (AOR 1.99, 95% CI: 1.00-3.98), all controlling for maternal age, race/ethnicity, marital status, education, health insurance status, income, and number of children in the household. Significant racial and ethnic disparities exist for pregnancy complications and non-live births, but not for low birth weight. Women’s preconception mental health is a modifiable risk factor that stands to reduce the incidence of adverse pregnancy complications and birth outcomes.
